This paper provides insight into the reasons underlying medical students' interest in geriatrics. Semi-structured interviews, informed by attitude theory, were conducted with first-year medical students who indicated that they were not interested in geriatric medicine (n = 10) and those who indicated that they were moderately to very interested in geriatrics (n = 10). The interviews were analyzed qualitatively. While most students in both groups did not plan on pursuing geriatric medicine, differences were found between the two groups. Students who expressed an interest in geriatrics had more positive and negative experiences with older adults, were more comfortable with palliative care, did not view their patients as responsible for their illnesses, and held more fears about aging and death than did those students who were not interested in geriatrics. The discussion of the results focuses on the role of experience in students' interest in geriatrics, and their beliefs about personal gain and loss and the field of medicine in general. Implications for educators, including the need to increase exposure to older patients and geriatric medicine for all students, are discussed.  相似文献

Medical care in nursing homes is not provided by board-licensed geriatricians; it mainly comes from physicians in need of educational programs in the field of geriatrics. Such programs, based on curriculum guidelines, should be developed. The purpose of this study was to seek input from nursing home physicians on their perceived needs for training in geriatrics. A mail questionnaire survey was sent to nursing home physicians regarding their opinion on the most needed subjects and preferred training methods. Of the 210 surveys mailed, 132 (63%) were returned. More than a quarter of the respondents had not had any kind of geriatric medical education. A desire for geriatric training was evident, preferably in the form of courses and periodic seminars. Use of medications, infectious diseases, depression, dementia and cardiac disorders were the most important topics indicated by the respondents. These data can be of help in preparing the curriculum for a continuous medical education program in geriatrics, preferably in the form of courses and periodic seminars.  相似文献

A lack of knowledge in registered nurses about geriatric conditions is one of the major factors that contribute to these conditions being overlooked in hospitalized older people. In China, an innovative geriatric continuing nursing education program aimed at developing registered nurses' understanding of the complex care needs of hospitalized older people with common geriatric conditions was conducted and evaluated. The program consisted of didactic sessions focused on evidence-based practice and unfolding case studies designed to simulate the care trajectory of an older person with a hip fracture and key geriatric conditions. Findings from the program evaluations revealed a significant increase in favorable attitudes towards older people and increased knowledge concerning common geriatric conditions. The satisfaction rate ranked by program participants was 90%. The study, therefore, drew the conclusion that effective geriatric continuing nursing education should target participants' learning needs, support evidence-based practice, and engage participants in active learning.  相似文献

Teaching geriatric principles to medical students is important because of the increasing geriatric population. Family physicians are in a strategic position to provide this teaching because they provide much of the care for the elderly. The challenge in developing a geriatric curriculum is to determine the content, implement it efficiently, and evaluate its impact. We developed a geriatric assessment experience for third-year medical students. The students were introduced to a geriatric assessment tool and asked to identify a patient, complete the assessment, and record key elements of functional assessment. Student attitudes were measured before and after completing the assessment. The students rated their knowledge, comfort, and experience with a comprehensive geriatric assessment. Students listed approximately seven problems and four recommendations per patient. On average, students identified one functional problem for each patient. There were significant increases in knowledge, comfort, and experience with a comprehensive geriatric assessment. This educational tool met all of our goals. The students identified elderly patients' functional problems, created care plans focused on function and prevention, and reported an increase in positive attitudes about their knowledge, comfort, and experience with geriatric assessment. No outside expertise was needed and faculty time demands were limited.  相似文献

Geriatrics education at the University of Valle Medical School uses many teaching strategies in undergraduate and postgraduate programs. These include improving communication with older patients, accepting one's own aging, teaching human values, providing experiences with well elderly, and participating in an interdisciplinary team. There are common curricular areas in geriatrics for health care disciplines which include medicine, dentistry, nursing, speech therapy, physiotherapy, and occupational therapy. However, each discipline also has specific curricular components. There is a curricular program for family medicine and internal medicine residents. A curriculum design was approved for a new geriatrics fellowship program which is in development. Finally, the progression of the curriculum is according to the learning needs of each level of education among medical students, residents and fellows.  相似文献

Continuing education programs for health care professionals who practice in geriatrics are an important part of improving the health care provided to older adults. Programs utilizing active forms of learning that mimic the clinical environment are more successful at changing the behavior of health care professionals than traditional didactic styles of teaching. Problem-based learning methods allow learners to identify their own areas of strength and weakness and to work toward improvement in a manner best suited to their needs. This article describes an interdisciplinary team of educator-clinicians in geriatrics who developed clinical case studies embodying these approaches as one method of improving the learning process for adult health care providers. An actual sample case study is presented as an illustration of the principles embodied in this process. Lessons learned from the development and use of these case studies are summarized in the context of improving the quality of continuing education programs for health care professionals in geriatric practice.  相似文献

This study examined first-year medical student attitudes concerning the elderly before and after instituting a geriatric mentoring program. The program began and ended with a survey designed to assess students' attitudes toward the elderly. During the mentoring program, students visited the same senior for four visits throughout the academic year. After each visit, students were required to write a narrative that included reflections on assigned themes. Comparisons of pre- and postsurvey data indicated that students became more positive in their attitudes toward older patients after the geriatric experience. Narratives indicated students were highly engaged in the visits with their seniors.  相似文献

Using the translated contents of the National Institute on Aging (NIA)'s Aging IQ, an educational intervention was delivered to older Korean Americans. The educational program was delivered via two different modalities, Internet-based education (n = 12) and in-class education (n = 11), and the overall feasibility and efficacy were evaluated by the Reach, Efficacy, Adoption, Implementation, and Maintenance (RE-AIM) framework. A high level of completion and overall satisfaction with the program was observed in both groups. Both groups also exhibited a significant improvement in their knowledge about aging after the completion of the educational program. However, at the one-month follow-up, the Internet-based group showed a greater retention of the knowledge gained. The findings not only support the value of education on aging for older ethnic minorities but also suggest the use of the Internet as an integral modality of intervention delivery.  相似文献

Each of the fields of geriatrics and interdisciplinary practice intensifies the usefulness and effectiveness of the other. Combining geriatrics and interdisciplinary practice also magnifies the complexity of two singularly complex fields. However, the subtle reasons for their complexity may escape the understanding of clinicians, educators, policy makers, and administrators. Attempting to treat older patients who have complex problems while ignoring common principles of geriatrics and teamwork can cause a kind of double jeopardy. It can potentially harm the patient and decrease the provider's sense of mastery. Training either students or health care providers to appreciate the interactions and the complexities of these two fields could produce a double indemnity for the health care system and for older persons who need health care. It requires recognizing subtle factors like the barrier of presumed knowledge, the interpretation of the meaning of function, frailty, the lure of medical technology, professional hierarchies, diverse views of disease, and the myth of insufficient evidence.  相似文献

The human papillomavirus (HPV) vaccine provides physicians with an opportunity to have conversations with girls about sex and sex-related topics. Current research suggests that these conversations are not happening. This study was designed to assess whether physicians would use the HPV vaccination as an opening to communicate with nine-year-old to 15-year-old female patients about sex using the Theory of Planned Behavior as a framework. The study also assessed what sexual health topics physicians were willing to discuss when they vaccinated patients and at what ages they would be willing to discuss each topic. Differences in communication were also examined between older and younger physicians and female and male physicians. A random sample (n = 207) of all family practice and pediatric physicians in a rural, Midwest state in the USA were mailed a survey about the HPV vaccine and their intentions to talk about sex-related topics. Specifically, the survey questions assessed the theoretical constructs: attitudes, subjective norms, and perceived behaviour control. The results point to physicians' intentions to talk about sex when they vaccinate. Most physicians intended to talk about sexually transmitted infections when they vaccinate against HPV (90.3%). A multivariate linear regression reveals that physicians' intentions to talk about sex are influenced by attitudes (β = 0.18, p < 0.05), subjective norms (β = 0.53, p < 0.001), and perceived behavioural control (β = 0.15, p < 0.05). Physicians intended to talk about a variety of sex-related topics when they vaccinate nine-year-old to 15-year-old girls against HPV. The research indicated that physicians' intentions were high, but do not indicate how well physicians are prepared for this communication with nine-year-old to 15-year-old girls about sex.  相似文献

Seasoned geriatric community health workers offered valuable data that will inform educators and community health workers working with older persons. Geriatric community health workers’ views about retirement and about facilitators and barriers that keep them engaged with older adults during retirement were identified. Twenty geriatric community health workers participated in semi-structured, qualitative interviews conducted by the research team. Sixty percent of the participants (N = 12) reported plans not to work during retirement, although 40% (N = 8) expressed interest in full- or part-time employment post-retirement. When asked about their expectations to stay in the field of aging or gerontology in some capacity during retirement, 30% (N = 6) expected to stay in gerontology during retirement. The findings from this research will help organizations and educational institutions create programs and resources that will encourage geriatric community health workers to continue working in the field of aging during retirement.  相似文献

Social work students have reported in previous studies that they receive insufficient coursework and training to work effectively with older adults. A critical factor in these deficiencies may be the level of knowledge of social work faculty. This study sought to assess social work faculty's knowledge of aging using the Knowledge of Aging for Social Workers (KASW) quiz. Using systematic random sampling, schools of social work in the United States were selected, and individual faculty members were invited by e-mail to participate in an online survey. Results show that social work faculty's (N = 609) knowledge about aging was found to be either less or comparable to other study populations. Knowledge levels were related to having an interest in policy, educational level, teaching aging courses on a regular basis, and confidence level in covering aging content. This study is the first to investigate social work faculty knowledge about aging and older adults. It may offer insight as to faculty competencies, which can have direct implications for student learning and interest in gerontology.  相似文献

As society ages, there is an increasing demand for knowledge and skills to care for the elderly. Innovative ways to stimulate the interest of medical students in geriatric medicine present an important challenge to medical schools today. This article discusses the value of a student interest group in meeting this challenge. In 1988, the Department of Geriatrics at the Mount Sinai School of Medicine in New York City initiated a student interest group in geriatrics. Over the ensuing years, this group has provided medical students with a forum for pursuing research interests in geriatrics, networking with other students, and interacting with faculty role models interested in geriatric medicine.  相似文献

This paper describes a Geriatric Health Professionals Mentoring Program designed to address recruitment and retention of health professionals in geriatrics and gerontology. The training provided information on the mentoring process, negotiating mentoring agreements, and coaching mentees. The evaluative framework described examines: (a) the effects of mentoring, (b) reactions of mentors and mentees, and (c) the effect of intervening variables. Trained mentors expressed satisfaction with the program and were comfortable about general mentoring but less confident about specific guidance for mentees. A cadre of mentors can be successfully trained to assist geriatric health professionals to better care for older Americans.  相似文献

The current study aimed to examine teachers’ reported spelling assessment and instruction practices. Analysis of the match between teachers’ theoretical beliefs about spelling and their reported pedagogy was conducted to elucidate factors that may support or impede the use of evidence-based teaching strategies in the classroom. An electronic survey was completed by 405 randomly selected (stratified by region and socioeconomic status) elementary school teachers in New Zealand. The survey examined the following areas: spelling assessment, spelling instruction, beliefs about spelling, preparing teachers to teach spelling, and teachers’ perceived strengths and weaknesses of their spelling program. There was large variability in spelling assessment and instructional practices across teachers. Most respondents reported implementing some aspects of a developmental approach to spelling instruction through analysis of children’s spelling errors (64 %) and/or individualization of the spelling program (60 %). There was a large dissociation between teachers’ beliefs about spelling and their frequency of use of specific instructional practices associated with those beliefs (e.g., phonological awareness, orthographic knowledge). The mismatch between beliefs and reported practice appeared to be due to lack of professional knowledge regarding implementing explicit spelling instruction and finding time to teach spelling within the curriculum. Increasing teachers’ knowledge about language structure, practical implementation of key assessment and instruction activities, and the links between spelling and other areas of the curriculum are important factors in improving spelling pedagogical practices.  相似文献

During the coming years the number of older adults will increase dramatically, which will require a corresponding increase in the number of social workers who have accurate knowledge of this population. This article reports the impact focus groups with older adults and with service providers who work with older adults and their families have on baccalaureate social work (BSW) students. We find that BSW students exposed to focus groups have their negative attitudes about older adults and service providers deconstructed and replaced with more positive and accurate information about older adults. When students' misconceptions about older adults are confronted, they become more interested in choosing geriatric social work as a career option.  相似文献

The Science Teachers Learning from Lesson Analysis (STeLLA) project is a videobased analysis‐of‐practice PD program aimed at improving teacher and student learning at the upper elementary level. The PD program developed and utilized two “lenses,” a Science Content Storyline Lens and a Student Thinking Lens, to help teachers analyze science teaching and learning and to improve teaching practices in this year‐long program. Participants included 48 teachers (n = 32 experimental, n = 16 control) and 1,490 students. The STeLLA program significantly improved teachers' science content knowledge and their ability to analyze science teaching. Notably, the STeLLA teachers further increased their classroom use of science teaching strategies associated with both lenses while their students increased their science content knowledge. Multi‐level HLM analyses linked higher average gains in student learning with teachers' science content knowledge, teachers' pedagogical content knowledge about student thinking, and teaching practices aimed at improving the coherence of the science content storyline. This study investigated the influence of professional development on 20 preschool teachers' use of instructional conversations in classrooms for 2- to 5-year-old children. Instructional conversation is a small group discussion that builds on children's prior knowledge to assess and assist conceptual understanding. Over a 3-year period, the educators participated in professional development designed to increase their use of the strategy in their classrooms. Each teacher was video-recorded six times annually while teaching. Two coders rated the video recordings, and the data were analyzed using a longitudinal multilevel model. Results indicated that regardless of their prior teaching experience, all of the teachers increased their use of instructional conversation throughout their participation in the professional development program. These increases were along the same trajectory for all teachers, suggesting that the professional development was effective for both new and veteran teachers. Results suggest that those who taught older children tended to use instructional conversation to a greater degree, indicating that the strategy may be easier to implement with older preschoolers.  相似文献

Although the impact of Alzheimer’s disease training programs directed to informal and formal caregivers has been extensively studied, programs for older adults who do not have the disease are relatively few. Moreover, increased knowledge increases fear of the disease, even though there is little empirical evidence to support this. This study investigated the impact of an Alzheimer’s disease training program for older persons on their knowledge and fear of the condition. Participants consisted of 110 older adults who voluntarily applied for a 16-hour course as part of the University of the Third Age. A questionnaire, consisting of two validated tools (Alzheimer’s Disease Knowledge Scale and the Fear of Alzheimer’s Disease Scale), was distributed pre- and posttraining to the participants. Sixty-six questionnaires (response rate = 60%) were collected at the start of the program and 46 at the end (response rate = 42%). Participants’ fear at pretraining was significantly related to their current knowledge of the disease. Training significantly improved knowledge of the disease, but the participants’ perceived fear did not change. The study indicated that an Alzheimer’s disease training program for older persons can increase their knowledge, but it does not necessarily increase their fear. Training programs aimed at raising awareness of Alzheimer’s disease should target more effectively this age cohort who may be concerned and are seeking learning opportunities to know more about the condition.  相似文献

The societal view that regards geriatrics as a period of decline may not be congruent to the views of elderly people themselves. This gives rise to the need for further understanding of how elderly individuals evaluate their lives. A phenomenological design was employed for purposes of discovering and characterizing the essence behind the life experiences and disposition of a select group of Filipino elderly and its role in shaping their attitudes toward death. Five individuals aged 65 and above were purposively chosen to participate. Interviews were tape-recorded, transcribed, and examined, applying both processing and scrutiny techniques. A set of themes representing the elderly's lived experiences was surfaced through warm and cool analyses. The study revealed that geriatrics is a period of intersection of the three dimensions of elderly's lived experience—reminiscence, living disposition, and end-of-life views. The respondents viewed their families as their main source of support and guidance, values, principles, and beliefs. Accepting problems as integral to life, they have found a driving force that made them surpass these challenges. New beliefs and attitudes were acquired, resulting in providing self-control, strength, appreciation, genuine spirituality, gratitude, fulfillment, and self-worth. In their end-of-life views, dignity was a major issue which must be recognized and reinforced prior to embracing one's destiny to die. The present study maintains that the three dimensions of elderly's lived experiences are equally important in geriatric care practice. This offers valuable insights and reflections for a better understanding of geriatrics as a neglected field in the health-care delivery system.  相似文献
