测量居民的健康-疾病负担,关键步骤是将健康-疾病负担的多维状态转化成可量化的内容,最终表达为一个可直接比较的综合性衡量指标,并形成国际共识的标准方法。使用伤残调整期望寿命(Disability-adjusted life expectancy,DALE)和伤残调整寿命年(Disability—adjusted life year,DALY)来测量健康状况和疾病负担已逐步成为国际共识。在健康-疾病负担测量的基础上进行医疗保险精算研究,所要解决的问题包括疾病发生风险的估计、医疗保险定价、偿付能力估计、盈利能力估计、新险种开发等。疾病发生次数的分布形式是确定总医疗费支付的一个重要因素,另一个重要因素就是每次发生费用的分布,即损失分布。住院医疗保险是商业医疗保险的主要形式,而设计和开发重大疾病以及高额费用医疗险种,又是医疗保险精算人员的主要任务。在住院医疗保险的纯保费测算中,按保险合同规定确定预期赔付额是其关键技术。基本的精算过程是在拟合出已知住院费用和住院天数的分布(即医疗保险的损失分布)后,用矩估计、变换的矩估计、极大似然估计和基于贝叶斯理论的蒙特卡洛马尔柯夫链(Markov chain Monte Carlo,MCMC)等方法来估计预期住院费用和预期住院天数。  相似文献

Although many authors claim benefits of teaching portfolios for use in higher education settings, there are few empirical studies that investigate these benefits.This article deals with the question of whether teaching portfolios are used in higher education, and if so, which effects they bring about. Furthermore, the attitude of teachers towards the use of teaching portfolios is examined. The study shows that currently not very many teachers are using teaching portfolios. When used, a teaching portfolio is an instrument that can bring about some important positive effects. Respondents report that, due to the use of portfolios, they were stimulated to reflect on their own teaching, to actualise the learning content, to improve their course materials, to search for alternative educational methods, etc. When teachers are using teaching portfolios it is important that, besides any negative effects, they also experience positive effects. If this is not the case, teachers will see the teaching portfolio only as an extra administrative inconvenience.  相似文献

This article reports on the interrater reliability of language arts portfolio assessment in the primary grades of the Rochester, New York, school system. This assessment was based on a sample of approximately 400 primary-grade language arts portfolios that were rated by two raters: the student's classroom teacher and an external reviewer. Reliabilities for student portfolios ranged from .58 for Grade 1 reading portfolios to .79 for kindergarten reading portfolios. Many of the reading portfolios could not be rated by the external reviewer, revealing problems both with the portfolio policies of the district and with the classroom-level implementation. The findings showed that primary-grade classroom teachers can reliably rate their own students' work, but tensions exist if a single portfolio system is intended both to provide classroom teachers with information about their students and to supply accountability information to administrators.  相似文献

The paper examines the role of PISA in the globalisation of education policies. It approaches the question by assessing the effects of PISA on the ways in which new legislation was debated in national contexts in the period 1994–2013. The study asks: Has there been an increase in the number of references to the international community in debates on education policy due to PISA, and, if so, is this change confined to debates on education policy? Our analysis shows that education policy debates feature an increasingly global discourse in which organisations such as the OECD have an authoritative role. Yet, our findings do not support the claim that PISA is the cause of a change in this respect. Debating national policies in a global context and utilising the same transnational discourses regardless of the policy issue area in question has long been with us, yet there is a global trend in which national policies are increasingly often debated through appeals to models and policy advice promulgated by international organisations.  相似文献

Abstract

During the last ten years universities in The Netherlands have become less dependent on national policies regarding salary structures, promotion criteria and reward systems. At the same time the government has put increased pressure on universities to take a serious look at the quality of education in general, and at the teaching qualities of the staff in particular. The effects of these policies on staff development at Dutch universities is described. The main focus is on recent initiatives by two universities, Utrecht and Maastricht, to improve the preparation of university teachers. For Utrecht a closer look is taken at the career system, in which the importance of teaching competence is upgraded; at the teacher training programme; and at the role of portfolios and assessment in the programme. This paper forms part of an exploration of assessment on one part‐time higher education (HE) course: an in‐service, professional qualification for teachers and trainers in the learning and skills sector which is delivered on a franchise basis across a network of further education colleges in the north of England. This paper proposes that the validity and reliability of portfolio‐based assessment, a key component of many HE programmes in addition to the course being researched here, is contestable. Analysis of the processes of compiling portfolios for assessment, through the conceptual framework of the New Literacy Studies, suggests that the ways in which portfolios are assessed and the ways in which the crucial requisites of validity and reliability are assigned to them, mask complexities and contradictions in their creation by the student. This paper argues for a new, critical analysis of portfolio production and raises a number of questions about the validity, reliability and authenticity of the assessment process that the portfolios reify.  相似文献
