The development of metabolic acidosis during cardio-pulmonary bypass (CPB) is a well recognized but poorly explained phenomenon. It has been hypothesized that it is purely a development after the delivery of pump prime. A retrospective study was conducted at our hospital on 68 patients who underwent elective coronary artery bypass grafting (CABG). Sampling of arterial blood was performed at three time intervals: (T1) Baseline, prior to induction; (T2) 5 minutes after initiation of CPB and prior to administration of cardioplegia solution; (T3) during rewarming prior to weaning the patient off CPB. Measurements of Na+, K+, Cl, pH, pCO2, HCO3 , Base excess, Anion gap, Strong ion difference at each collection point were performed. Results were analyzed in a quantitative manner. On delivery of pump prime, all patients' developed metabolic acidosis. However, it is very important to distinguish the metabolic acidosis as their management varies. Anion gap has been found to be useful in managing peri-operative metabolic acidosis.  相似文献

Cardiopulmonary bypass is known to cause alterations in insulin secretion and resistance, resulting in profound hyperglycemia. Aggressive treatment of the resulting hyperglycemia intra-operatively could result in a severe degree of post-operative hypoglycemia. We undertook this prospective non-randomized clinical study to compare the alterations in glucose homeostasis in diabetic (group A, n=50) and non-diabetic (Group B, n=50) patients undergoing moderate hypothermic (30°C) cardiopulmonary bypass for coronary artery bypass grafting (CABG). All patients had a fasting blood sugar level done on the morning of surgery. Blood sugars were monitored intra-operatively and post-operatively at fixed time intervals. Intra-operative hyperglycemia was treated aggressively by a continuous, infusion of injecting plain insulin. Both the groups experienced similar significant increase in blood glucose levels during bypass (‘p’=0.00003). However, the mean blood glucose level upon arrival in the intensive care unit was significantly decreased in group B compared to group A (p=0.0002). 60% of group B and 10% of group A patients required treatment for post-operative hypoglycemia (blood glucose level <60mg/dl). This clinical study reveals that attempting to maintain normoglycemia in this setting with Insulin may initiate post-operative hypoglycemia.  相似文献

As the oxygen tension of inspired air falls with increasing altitude in normal subjects, hyperventilation ensues. This acute respiratory alkalosis, induces increased renal excretion of bicarbonate, returning the pH back to normal, giving rise to compensated respiratory alkalosis or chronic hypocapnia. It seems a contradiction that so many normal people at high altitude should permanently live as chronic acid–base patients. Blood gas analyses of 1,865 subjects at 3,510 m, reported a P aCO2 (arterial carbon dioxide tension ± SEM) = 29.4 ± 0.16 mmHg and pH = 7.40 ± 0.005. Base excess, calculated with the Van Slyke sea level equation, is −5 mM (milliMolar or mmol/l) as an average, suggesting chronic hypocapnia. THID, a new term replacing “Base Excess” is determined by titration to a pH of 7.40 at a P aCO2 of 5.33 kPa (40 mmHg) at sea level, oxygen saturated and at 37°C blood temperature. Since our new modified Van Slyke equations operate with normal values for P aCO2 at the actual altitude, a calculation of THID will always result in normal values—that is, zero.  相似文献

Although potassium is critical for normal electro physiology, the associations between pre-operative serum potassium level and peri-operative adverse events such as arrhythmias in cardiac surgery have not been examined in detail. The objective of this study was to determine the prevalence of abnormal pre-operative serum potassium levels and whether such levels were associated with adverse peri-operative events in 50 patients undergoing coronary artery bypass grafting. Intra-operative and post-operative arrhythmias, the need for cardio-pulmonary resuscitation, cardiac death and death due to any cause prior to discharge from the post-operative intensive care unit were studied. The incidence for adverse outcome was 0.5% for death, 0.5% for cardiac death and 2% for cardio pulmonary resuscitation in patients with hypokalemia (serum potassium level <3.5 meq. L−1). Hypokalemia was found to be a predictor of serious peri-operative (OR:2.2; 95% Cl: 1.2–4.1) and post-operative arrhythmias (OR: 1.7;95%Cl: 1.0–2.7). Pre-operative potassium repletion is low cost and low risk treatment measure and the data from this study suggests that screening and repletion be considered in patients scheduled for cardiac surgery.  相似文献

Partial pressure of oxygen (PO2) was estimated at various intervals during conventional Acetate hemodialysis (A), Sequential haemodialysis (SHD) i.e. ultrafiltration followed by haemodialysis and Isolated ultrafiltration (IUF) on 35 patients. Study was extended for PO2 measurement one hour after termination of above treatment. There was a significant lowering of PO2 observed at 1 h. during ‘A’ (93.3±10.5 Pre to 78.8±4.2 post, P<0.01) during SHD, a significant fall was observed after the dialysate flow was started (81.1±11.5 Pre to 76.3±11.8, 2hr P<0.05) at second hour of the procedure. IUF showed significant increase (88.4±10.3 pre, to 100.4±12.4, post, P<0.01) at the end of treatment. A significant rebound increase was observed after one hour of termination of ‘A’. Changes observed after termination of ‘SHD’ and ‘IUF’ were not significant. The results suggest that, though dialyzer membrane lowers the PO2 level, major role is played by the dialysis fluid.  相似文献

Nineteen subjects of non-insulin dependent diabetes mellitus (NIDDM) between the age group of 30–60 yrs were studied to see the effect of specific yoga asanas on fasting and postprandial blood glucose (FBG, PPG), serum malondialdehyde (MDA) and glycosylated hemoglobin (HbA1) in addition to drug treatment and diet control. The duration of diabetes ranged from 1–10 years. Patients with renal, cardiac and proliferative retinal diseases were excluded from the study. The same patients served as their own control. Subjects were called in the morning to the cardio-respiratory laboratory and were given training by a yoga expert. Yoga asanas included Suryanamskar, Tadasan, TriKonasan, Padmasan, Pranayam, Paschimottanasan, Ardhmatsyendrasan, Pavanmukthasan, Sarpasan and Shavasan. The asanas were done every day for 40 days for 30–40 min. FBG, PPG, serum MDA and HbA1 were estimated before and after 40 days of yoga asanas regimen. Significant reduction was seen in FBG from 220 mg/dl to 162 mg/dl, PPG from 311 mg/dl to 255 mg/dl, MDA from 6 nmol/l to 3 nmol/l and HbA1, from 8.8% to 6.4%. Subjects felt better and were relieved of their stresses and had an improvement in their day to day performance. The decrease was statistically significant (p<0.0001 for FBG and PPG, p<0.001 for MDA and for HbA1).  相似文献

The study was designed to evaluate the antioxidant activity and effect of Cymbopogon martinii (Roxb.) Wats. (Poaceae) leaves on the activity of monoamine oxidase and kinetics of enzyme inhibition. Ethanol extract of C. martinii and rat brain mitochondrial monoamine oxidase preparation ware used to study the kinetics of enzyme inhibition using double reciprocal Lineweaver–Burk plot. The DPPH was used as a source of free radical to evaluate antioxidant potential. It is observed that, the ethanolic extract of C. martinii inhibits the monoamine oxidase activity with competitive mode of inhibition. The V max (0.01 mM/min) remained constant while, K m varied from 21.00 ± 1.1, 43.33 ± 1.5 and 83.33 ± 1.4 mM for 100–500 μg/ml concentration of C. martinii. The K i values were calculated to be 90.00 ± 0.87, 75.00 ± 0.69, 68.18 ± 0.68 μg for 100–500 μg/ml concentration of C. martini. It also shows a significant DPPH (1,1-diphenyl-2-picryl hydrazine) radical scavenging (IC50 = 0.34 ± 0.05 mg/ml) and reducing activity (IC50 = 0.70 ± 0.22 mg/ml). The C. martini can be considered as a possible source of MAO inhibitor used in the treatment of depression and other neurological disorders.  相似文献

BackgroundIron magnetic nanoparticles have attracted much attention. They have been used in enzyme immobilization because of their properties such as product is easily separated from the medium by magnetic separation. The present work was designed to immobilize horseradish peroxidase on Fe3O4 magnetic nanopraticles without modification.ResultsIn the present study, horseradish peroxidase (HRP) was immobilized on non-modified Fe3O4 magnetic nanoparticles. The immobilized HRP was characterized by FT-IR spectroscopy, scanning electron microscopy, and energy dispersive X-ray. In addition, it retained 55% of its initial activity after 10 reuses. The optimal pH shifted from 7.0 for soluble HRP to 7.5 for the immobilized HRP, and the optimal temperature shifted from 40°C to 50°C. The immobilized HRP is more thermostable than soluble HRP. Various substrates were oxidized by the immobilized HRP with higher efficiencies than by soluble HRP. Km values of the soluble and immobilized HRP were 31 and 45 mM for guaiacol and 5.0 and 7.0 mM for H2O2, respectively. The effect of metals on soluble and immobilized HRP was studied. Moreover, the immobilized HRP was more stable against high concentrations of urea, Triton X-100, and isopropanol.ConclusionsPhysical immobilization of HRP on iron magnetic nanoparticles improved the stability toward the denaturation induced by pH, heat, metal ions, urea, detergent, and water-miscible organic solvent.  相似文献

BackgroundCyclodextrin glucanotransferase (CGTase) from Amphibacillus sp. NPST-10 was covalently immobilized onto amino-functionalized magnetic double mesoporous core–shell silica nanospheres (mag@d-SiO2@m-SiO2-NH2), and the properties of the immobilized enzyme were investigated. The synthesis process of the nanospheres included preparing core magnetic magnetite (Fe3O4) nanoparticles, coating the Fe3O4 with a dense silica layer, followed by further coating with functionalized or non-functionalized mesoporous silica shell. The structure of the synthesized nanospheres was characterized using TEM, XRD, and FT-IR analyses. CGTase was immobilized onto the functionalized and non-functionalized nanospheres by covalent attachment and physical adsorption.ResultsThe results indicated that the enzyme immobilization by covalent attachment onto the activated mag@d-SiO2@m-SiO2-NH2, prepared using anionic surfactant, showed highest immobilization yield (98.1%), loading efficiency (96.2%), and loading capacity 58 µg protein [CGTase]/mg [nanoparticles]) which were among the highest yields reported so far for CGTase. Compared with the free enzyme, the immobilized CGTase demonstrated a shift in the optimal temperature from 50°C to 50–55°C, and showed a significant enhancement in the enzyme thermal stability. The optimum pH values for the activity of the free and immobilized CGTase were pH 8 and pH 8.5, respectively, and there was a significant improvement in pH stability of the immobilized enzyme. Moreover, the immobilized CGTase exhibited good operational stability, retaining 56% of the initial activity after reutilizations of ten successive cycles.ConclusionThe enhancement of CGTase properties upon immobilization suggested that the applied nano-structured carriers and immobilization protocol are promising approach for industrial bioprocess for production of cyclodextrins using immobilized CGTase.  相似文献

An Oxalate oxidase (Oxalate: O2 oxidoreductase, EC 1,2,3,4) has been purified to apparent homogeneity from leaves of 10-day old seedling plants of grain sorghum hybrid CSH-5. The enzyme exhibited maximum activity at pH 5.0 and 40°C. The rate of H2O2 formation was linear upto 2 min. The enzyme was strongly stimulated by Cu++. The enzyme has greater resistance towards various cations and anions found in urine, compared to moss, barley, banana peel and bleet stem oxalate oxidases. This improved characteristic of the enzyme make it better suited for its use in the determination of urinary oxalate. A simple method of measuring oxalate in urine using this enzyme preparation is described.  相似文献

The present study was conducted to study the significance of lactate as a prognostic marker in patients of septic shock with acute respiratory distress syndrome. This study was conducted on 50 critically ill patients of septic shock with acute respiratory distress syndrome between the age group of 20–60 years and 50 controls. Plasma lactate and serum electrolytes were determined among controls and patients. Arterial blood gas analysis for pO2, pCO2 and pH was carried out among patients. Arterial base excess and anion gap were calculated and lactate was correlated with base excess, anion gap and pCO2 at 5% level of significance. Higher lactate, negative arterial base excess, high anion gap, low pO2 and high pCO2 were observed among patients. Lactate was positively correlated pCO2 and anion gap and negatively with pO2 and base excess among patients. Hyperlactatemia increasing with progression of septic shock with acute respiratory distress syndrome may suggest that lactate may be used as noninvasive prognostic marker or guide to resuscitation.  相似文献

Using the velocity analyzer of Zartman with improved technique the combined velocity spectrum of Bi atoms and Bi2 molecules was obtained at 827°, 851°, 875°, 899°, 922°, 947° C. From the spectral distribution curves the relative abundance of Bi atoms and Bi2 molecules in the beams at the above temperatures could be determined to 1 per cent. The vapor pressure curve of Bi was obtained experimentally by the method of effusion and the values so obtained were combined with the degree of dissociation of the vapor as computed from the beams to give the heat of dissociation. The heat of dissociation was computed from the data, assuming the pressure to be given by the temperature of the crucible Tc. In calculating the heat of dissociation, the equilibrium temperature was taken as that of the slit chamber Ts which was 24° above Tc. The results of these calculations plotted with log10Kp as ordinates against 1Ts give a straight line whose slope yields the value of the heat of dissociation as 77,100±1200 calories. The curves for the distribution of velocities observed and computed on the assumption of a given ratio of Bi atoms to Bi2 molecules in the beam were compared in an attempt to test the law of distribution of velocities. On the high velocity side agreement in two curves was obtained within the limits of experimental accuracy. On the low velocity side important deviations were noted of such a sort that the observed curves below a velocity α2, (α is the most probable velocity) gave more molecules than the theory demanded. Other deviations were observed on some of the runs taken with a fourth slit in which a deficiency of molecules was observed between velocities of .75α and α2. This deviation was probably due to a warping of the fourth slit carriage due to heat. The nature of the variation at velocities less than α2 indicated the presence of molecules of greater mass than Bi2 in the beam and at the lower temperatures a distinct peak corresponding to Bi8 molecules was observed which were present to less than 2 per cent. The vapor pressure curve for Bi was determined by least square reduction of the observed points to be given by log10 P = ? 52.23 × 195.26T + 8.56 between 1100° and 1220° abs. It lies very close to the extrapolated curve given in the International Critical Tables.  相似文献

Serum β2-microglubin (β2-m) levels were measured in oral carcinoma patients and compared with normal healthy controls. It was observed that there was a significant rise in serum β2-microglobulin in oral carcinoma patients. Progressively higher values were obtained as the cancer advanced clinically. Therefore the estimation of serum β2-microglobulin may be useful as one of a battery of tests in the assessment of oral carcinoma patients.  相似文献

In Ayurvedic medicine,Withania somnifera (Ashwagandha) is well known for its anti-stress activity. A passive rat experimental model, where the animals are subjected to multiple stress of cold, hypoxia, restraint (C-H-R) has been developed to evaluate adaptogenic properties of various fractions of W. somnifera root extracts. We have carried out extraction of roots of W. somnifera with water and further isolated one of the active constituents called compound X and also tested its anti-stress activity in C-H-R model. The effect of administration of W. somnifera water suspension (360 mg/Kg bw) and compound X (20mg/Kg bw) on the fall and recovery of colonic temperature was noted. There was an increase of ≈38% and ≈54% in the time taken to attain Trec 23°C by rats given a single dose of fresh aqueous suspension and biologically active constituent (Compound X) respectively, where as decrease in the recovery time to attain Trec 37°C is ≈13% and ≈33% respectively, as compared to control group. It is clear that rats treated with the fresh aqueous suspension and compound X ofWithania somnifera, could withstand the multiple stress of C-H-R better than control group.  相似文献

Glycosylated Hemoglobin (GHb) was estimated using cation exchange resin binding method in 50 non diabetic healthy controls and 500 maturity onset diabetics. Percentage of glycosylated hemoglobin levels in controls was 7.27±0.456 (range 6.0–8.04) and 9.47±0.98 (range 8.2–13.63) in diabetic group. Glycosylated hemoglobin levels remain unaffected inspite of transient rise of serum glucose levels during an oral glucose tolerance test. Significantly elevated GHb A1. Furthermore significant elevation of GHb A1 was noticed in diabetics with all secondary complications suggesting poor glycemic control in these patients. The diabetics having retinopathy, nephropathy or diabetic foot had relatively higher GHb levels compared to patients having coronary artery disease or hypertension.  相似文献

BackgroundLXYL-P1-2 is the first reported glycoside hydrolase that can catalyze the transformation of 7-β-xylosyl-10-deacetyltaxol (XDT) to 10-deacetyltaxol (DT) by removing the d-xylosyl group at the C-7 position. Successful synthesis of paclitaxel by one-pot method combining the LXYL-P1-2 and 10-deacetylbaccatin III-10-β-O-acetyltransferase (DBAT) using XDT as a precursor, making LXYL-P1-2 a highly promising enzyme for the industrial production of paclitaxel. The aim of this study was to investigate the catalytic potential of LXYL-P1-2 stabilized on magnetic nanoparticles, the surface of which was modified by Ni2+-immobilized cross-linked Fe3O4@Histidine.ResultsThe diameter of matrix was 20–40 nm. The Km value of the immobilized LXYL-P1-2 catalyzing XDT (0.145 mM) was lower than that of the free enzyme (0.452 mM), and the kcat/Km value of immobilized enzyme (12.952 mM s−1) was higher than the free form (8.622 mM s−1). The immobilized form maintained 50% of its original activity after 15 cycles of reuse. In addition, the stability of immobilized LXYL-P1-2, maintained 84.67% of its initial activity, improved in comparison with free form after 30 d storage at 4°C.ConclusionsThis investigation not only provides an effective procedure for biocatalytic production of DT, but also gives an insight into the application of magnetic material immobilization technology.How to citeZou S, Chen TJ, Li DY, et al. BackgroundHydroxycinnamic acids and some of their derivatives are molecules with interesting biological activities; for instance, hydroxylated hydroxycinnamic esters have proved to have antifungal properties, and thus the generation of these molecules is of industrial importance. In this study, the direct esterification capacity of the pure recombinant type B feruloyl esterase from Aspergillus terreus (AtFAE B) was evaluated by its ability to catalyze the synthesis of isobutyl o-coumarate, an interesting antifungal molecule. A ternary solvent system (isooctane/isobutanol/water) was employed to improve the synthesis of isobutyl o-coumarate, assessing different substrate concentrations, enzyme load, water percentages and pH and temperature values.ResultsAtFAE B showed the highest initial rate at 18% (v/v) isobutanol and 50 mM o-coumaric acid, 0.04 mg/ml of enzyme, 4% (v/v) water without buffer and 40°C. AtFAE B half-lives at 30°C, 40°C and 50°C were 16.5 h, 1.75 h and 3.5 min, respectively. Thus, we decided to evaluate the bioconversion yield at 30°C, where the enzyme showed the highest operational stability. At this temperature, we obtained a yield of ~80% after only 8 h of reaction, using a 78:18:4 isooctane:isobutanol:water ternary solvent system, with 50 mM of o-coumaric acid.ConclusionsUnder these improved conditions, the productivity was 1.06 g isobutyl o-coumarate/L*h with a biocatalyst yield of 209.6 kg isobutyl o-coumarate/kg free AtFAE B, demonstrating the promising potential of AtFAE B to accept the non-canonical o-coumaric acid as the substrate and to achieve the synthesis of isobutyl o-coumarate.How to cite: Vega-Rodríguez AD, Armendáriz-Ruiz MA, Grajales-Hernández DA, et al. Wistar rats of 6–8 weeks in age weighing between 120–150 g were exposed to the fixed doses of each of the carbamate pesticides such as cartap (50% LD50) and carbofuran (50% LD50) as well as a combination of these two with 25% LD50 of each for one week. The effect of treatments was studied in terms of serum lipid parameters such as high-density lipoprotein, total cholesterol, triglyceride, low-density lipoprotein and very low-density lipoprotein. Treatment with individual doses of carbofuran (50% LD50) and cartap (50 % LD50) caused significant alterations in the levels of serum lipid parameters. The pesticides treatment resulted in marked decrease in the level of serum high-density lipoprotein where as that of other lipids got significantly elevated. Further, the rats exhibited relatively higher impact of pesticides when treated with the compounds in combination (25 % LD50 of each). The results indicated that these compounds when used together may exert enhanced effect on the levels of serum lipids in rat.  相似文献

Graves’ disease (GD) is an organ-specific heterogenous autoimmune disorder associated with T-lymphocyte abnormality affecting the thyroid, eyes and skin. GD is a multifactorial disease that develops as a result of complex interaction between genetic susceptibility genes and environmental factors. It has been suggested that the Cytotoxic T lymphocytes associated molecule-4 (CTLA-4) is a genetic susceptibility candidate for GD. The present study was focused on A/G polymorphism at position 49 in exon-1 of the CTLA-4 gene in 80 GD patients (GP) and 80 sex and age matched healthy individuals among South Indian (Madurai) population. Serum concentrations of thyroid hormone (T4, T3 and TSH) were determined by using automated analyzer. The genomic DNA was isolated from the patient and control groups and genotyping was performed using the polymerase chain reaction followed by restriction enzyme analysis using Bbv1. Significant difference (P < 0.001) was observed in the level of T3, T4 and TSH in GD patients and healthy individuals. The results revealed the CTLA-4 gene G/G genotype to be 32 (40%) in patients and 26 (32.50%) in healthy individuals, A/G genotype to be 37 (46.25%) in patients and 25 (31.25%) in healthy individuals and A/A genotype to be 11 (13.75%) in patients and 29 (36.25%) in healthy individuals. The calculated odds ratio (OR) in individuals with mutant genotype (GG/AG) reveal 3.6 fold risk for GD (95% confidence interval = 1.6–7.8). The mutant “G” allele frequency was observed to be 0.63 in GD patients and 0.48 in healthy individuals. Thus the present study demonstrates an association between the CTLA-4 gene polymorphism and Graves’ disease.  相似文献

Diabetes mellitus and thyroid disorders are common endocrinopathies, which often occur parallel. Dyslipidemia is very common in both of these conditions. The development of hypothyroidism is well-known in type 1 diabetics, but it was not distinctly understood in type 2 diabetics. Thus we tried to examine the association between type II deiodinase (D2 or DIO2) Thr92Ala single nucleotide gene polymorphism and thyroid function among type 2 diabetes mellitus patients. A total of 130 type 2 diabetics were screened and genotyped for DIO2 Thr92Ala polymorphism. Fasting plasma glucose, Glycosylated haemoglobin, lipid and thyroid profiles, malondialdehyde (MDA) and paraoxonase were estimated according to standard procedures. A significant altered level of thyroid hormones (TH’s) was found in Ala/Ala genotype when compared with Thr/Thr or Thr/Ala genotype. DIO2 and T3:T4 ratio significantly decreased, whereas total T4 and thyroid stimulating hormone levels significantly elevated among Ala/Ala genotype (131 ± 30 ng/ml; 0.12 ± 0.05; 7.17 ± 2.05 µg/dl; 4.77 ± 3.1 µIU/ml, respectively) when compared with Thr/Thr + Thr/Ala genotypes (176 ± 33 ng/ml; 0.21 ± 0.05; 5.21 ± 1.1 µg/dl; 2.59 ± 1.61 µIU/ml respectively). Moreover, D2 levels were significantly negatively correlated with TH’s levels except total T4 among Ala/Ala genotypes. All the patients were having a poor glycemic control, and their glycemic status was positively correlating with MDA levels. On the other hand, serum paraoxonase activity decreased among Ala/Ala genotype (104 ± 21 vs. 118 ± 18 nmol/min/ml). In conclusion, DIO2 Ala92 homozygous variant found to be associated with altered levels of DIO2, Thyroid profile and paraoxonase. Hence, we recommend to do detail study of genetic factors related to thyroid function and prevent additional diabetic complications.  相似文献
