社区卫生服务是城市卫生工作的重点,是我国新型医药卫生体制改革与保证卫生事业持续发展的重要举措,也是实现"人人享有卫生保健"战略目标的一件大事。然而,卫生人力资源的数量和质量影响卫生服务的可及性和卫生服务的质量,构建一支适宜的社区卫生服务人力队伍是发展社区卫生服务最关键的因素之一[1]。因此,本文对潍坊市社区卫生服务机构人力资源配置情况及其主要影响因素进行问卷调查及统计分析,从而为卫生行政部门制定社区卫生服务的人力开发政策、医学高等院校制定相应人才培养规划和提高社区卫生服务质量提供现实依据。  相似文献

通过问卷调查与个别访谈相结合的方式,对石家庄市某社区卫生服务站进行了个案调查.结果表明,该社区卫生服务站的民众认可度较高,社区卫生服务功能基本得以体现.但服务站建设中还存在着一些问题,困扰着社区卫生服务工作的进一步开展.为此,一方面,需充分发挥政府主导作用,落实政府补偿机制,加大宣传力度,加强监督,落实保障措施;另一方面,应加强服务站自身建设,转变观念,完善管理机制,提高服务水平,创新服务内容,积极开展中医服务.  相似文献

ABSTRACT

As increased attention and proposed funding are being directed toward community colleges, it is important to consider the sexual and reproductive health care needs of this growing population. Existing data suggest there are significant sexual health needs among this population and often insufficient provision of services. Some community college students are more likely than students at 4-year colleges to test positive for sexually transmitted diseases (STDs). Given resource constraints, creative solutions are required. These may include campus-wide policies addressing STD/HIV (human immunodeficiency virus) prevention, referral systems to connect students to care in the community, and partnerships with local health departments, Federally Qualified Health Centers, or community-based organizations to assist with the provision of services. Colleges have the unique opportunity to provide students with valuable information about sexual health and services. Community colleges, in particular, are uniquely positioned to reach at-risk community members for STD testing and sexual health care who might otherwise be lost to care. More research is needed on the sexual health needs of community college students, especially on factors such as geographic location, how embedded the school is into the community, social norms around sexual health on college campuses, and health services offered. New and innovative ways to promote linkage to care for testing and counseling could offer potential health benefits to this growing at-risk population.  相似文献

亲属照护居家高龄失能老人仍是中国现阶段和今后较长时间内的主要照护形式。目前国内关于老年照护的研究集中于老年人本身的照护需求,而对亲属照顾者的照护状况、困难和需求研究不够深入。"高龄体弱老人照顾者需求和心理健康状况"课题组于2010年4—10月对上海市720名户籍城市居家高龄失能老人及其主要亲属照顾者所进行的抽样调查表明,目前上海居家高龄失能老人主要亲属照顾者的照护呈现女性居多、中老年退休者为主、经济条件适中、照护时间较长和照护责任心强的特点。照顾者在照顾过程中面对生理、心理和经济的压力虽然能够积极采取自我解压的方式,并从家庭获得支持,但同时对政府和社会提供支持也提出了迫切需求。为此,需要通过健全社会医疗保障体系,强化小区为基础的居家服务,加强照护知识技能培训,组建照顾者互助小组,完善以亲属照顾者需求为中心的社会支持系统。  相似文献

在日本、法国、英国、丹麦、澳大利亚、加拿大等国,家庭托育是其托育服务体系的重要组成部分,所以这些国家都形成了相对完善的家庭托育政策管理体系。从国际经验来看,虽然家庭托育的服务类型多样,但是纳入监管的主要是由有资质(注册)的照护人员在自己家(或租赁的住宅)中提供保教结合的服务内容的家庭托育点,政府责任以保障兜底服务为主,广泛使用了权威工具、激励工具和能力建设工具,包括明确行政主管部门与家庭托育点的准入标准和审批流程,给予财政和非财政支持,加强师资培训和质量监管,等等。当前虽然我国很多城市普遍兴起家庭托育点,但是尚未出台关于家庭托育点的管理规范与措施。借鉴国际相关公共管理经验,我国应构建符合利益相关者的家庭托育政策目标,合理运用政策权威工具,建立全方位安全监管体系,组合配置激励工具与能力建设工具,支持家庭托育点规范有序发展,以满足婴幼儿家庭对具备收托幼儿数量少、师幼配比高、收托时间灵活、送托距离近等诸多便利条件的家庭托育点的需求。  相似文献

The transition from maternity services to community child health services on discharge from hospital occurs at a potentially vulnerable time for women in their transition through the childbearing/early parenting continuum. Their experiences contribute to their developing maternal efficacy and parenting skill. The ideal attributes of services that aim to support women and their families during this time include continuity of care, service integration, and birth in accessible, community-based contexts. The purpose of this study was to investigate aspects of maternal experience of mothers attending with their infants a publicly funded drop-in postnatal health-care service, as well as their reasons for attending and their perceptions of its usefulness to them as a mechanism of continuity and a source of support.  相似文献

Professional practice with older adults is performed in a variety of settings and across a broad range of areas. Planning for care throughout the end of life represents an increasingly important aspect of work with older adults as a result of the nation's aging demographic and concomitant health care needs. Community-based geriatric case managers represent a growing cadre of professionals encountering frail older adults with future care needs that can be anticipated in advance; however, little is known regarding specific professional and personal factors associated with greater practice in this area. This article presents the findings from a study of 148 community-based case managers' advance care planning practices, conducted in the Southeastern United States. Correlational analyses revealed specific gerontological skills, educational activities, and personal experiences in advance care planning that was highly correlated with practice. Implications for training are discussed.  相似文献

This article presents some of the most salient qualitative results from a larger program evaluation of pregnant and parenting adolescents who participated in a community-based doula program. Using grounded theory analysis, seven problem-solving strategies emerged that doulas apply in helping pregnant and parenting adolescents navigate multiple social and health settings that often serve as barriers to positive maternal- and child-health outcomes. The ethnographic findings of this study suggest that the doulas provide valuable assistance to pregnant and parenting adolescents by addressing social-psychological issues and socio-economic disparities. "Diverse role-taking" results in doulas helping pregnant adolescents navigate more successfully through fragmented social and health service systems that are less supportive of low-income adolescents, who are often perceived to be draining scarce resources. The findings have implications for the roles of community-based doulas assigned to low-income adolescents of color seeking to overcome obstacles and attain better educational and economic opportunities.  相似文献

Health literacy has firmly established the links between literacy skills and health outcomes and is subsequently considered a key strategy for improving the health of disadvantaged populations and addressing social inequality. However, current research findings for improving health literacy have primarily focused on adults and actions within health and health care settings. Implementation studies outside the health sector are scarce. This study, a subset of the INCLUD-ED community- based project on social inclusion, reports on successful community-based approaches to health literacy. This article focuses on two schools that take advantage of the cultural intelligence of their students' family members, allowing them to make health literacy programmes more effective and useful for the participants. In addition, family involvement in educational activities addressed to children, including health programmes, has been found to improve the health literacy of the participating adults and their use of healthcare services. Findings indicate that schools in Europe can play a key role in breaking the cycle of health inequalities by promoting health literacy through education.  相似文献

The early childhood community-based program Hogares Comunitarios de Bienestar serves 800,000 low-income children under the age of 5 in Colombia. Earlier studies have identified care providers (madres comunitarias) as having, on average, low education levels and not being appropriately trained for the provision of childcare services. In 2007, a vocational education program was introduced which offers a degree in child development and care for childcare providers. In this paper we assess the effects of this program on the quality of care provided in the relevant nurseries and on the nutritional and health status, as well as cognitive and socio-emotional development of beneficiary children in Bogotá. This evaluation takes advantage of the gradual geographic expansion of the program in order to estimate treatment effects. Results indicate that the quality of care has significantly increased in these family-based nurseries and that the program has thus had a positive and significant effect on the health, cognitive and socio-emotional development of beneficiary children, especially those younger than 3 years of age.  相似文献

Death can be considered a social taboo, a common source of fear and public avoidance. School communities are not immune to this, as the topic of death is constantly avoided. It is vital to understand how we can socially and culturally cultivate a positive regard for death, dying and bereavement in our school communities. Community members need to discuss these difficult issues and use strategies to enhance compassion, connectedness and support. In this literature review we reason that death is specifically not ‘part of life’ in school communities. Due to the dearth of school community-based literature on this issue and the progressive literature residing in palliative care, we aim to coalesce palliative care and school-based research, evaluate it and highlight compassion and partnership as a way forward for school communities. Essentially, our societal attitudes about death and dying have been profoundly altered and our community ownership of these normal life events has largely disappeared. This is demonstrated for example, by palliative care moving from the social grass roots ‘modern hospice movement’ formed in the 1960s and being reintegrated into the mainstream health care system by the end of the 1990s, resulting in an overall medicalised morphing of death, dying and bereavement issues. Therefore, we recommend that further research be conducted in how to develop compassionate schools to inform us how death may be continually made ‘part of life’ in school communities, for the benefit of students, teachers and families alike.  相似文献
