Introduction:

The measurement of cardiospecific troponins is pivotal in the diagnostic and prognostic approach of patients with suspected acute myocardial infarction (AMI). However, no information is available on the commutability of results between the novel highly-sensitive (HS) troponin T (TnT) and I (TnI) immunoassays.

Materials and methods:

The study population consisted in 47 consecutive patients presenting at the emergency department (ED) of the Academic Hospital of Parma with suspected AMI. TnI was measured with the novel prototype Beckman Coulter HS-AccuTnI immunoassay on Access 2, whereas TnT was measured with the Roche HS-TnT immunoassay on Cobas.

Results:

Eight out of the 47 patients (17%) were finally diagnosed as having an AMI. The overall correlation between TnT and TnI for total patient group was acceptable (r = 0.944; P < 0.01). Nevertheless, when the analysis of data was carried out in separate groups according to the final diagnosis of AMI, two different equation results were obtained, i.e., HS-TnT = HS-AccuTnI × 0.349 + 20 (r = 0.823; P < 0.01) in non-AMI patients, and HS-TnT = HS-AccuTnI × 0.134 + 67 (r = 0.972; P < 0.01) in those with AMI.

Conclusions:

This study suggests the existence of two biological relationships between TnI and TnT in plasma, depending on the source of release from the myocardium. Moreover, the non-commutability of data between HS-TnT and HS-AccuTnI jeopardizes the clinical decision making, makes it impossible to calculate the delta or reference change value using the two biomarkers and to finally establish a reliable kinetics of troponin release from the injured myocardium.  相似文献

The use made of evaluation output is crucial for understanding the position and effectiveness of evaluation systems. This article examines the development of evaluation in the Italian university system from the 1990 s onwards where serious problems have been and continue to be addressed in the use of evaluation output to improve academic activities and make universities accountable for their behaviour. There is a tendency for evaluation to generate inappropriate or undesired effects and for those being assessed to adopt opportunistic behaviour. Underestimation of the universities’ organisational features is the main reason for so many difficulties.  相似文献

Multispectral imaging techniques are widely used to analyse and restore digital images of ancient documents degraded over time. In particular, acquisitions in infrared and ultraviolet bands can reveal information invisible by naked eye, which is not captured by conventional RGB imaging. Multispectral acquisitions are usually performed with filter-wheel cameras that mount a series of interference filters in front of the sensor to select the spectral band in which the document has to be acquired. Due to the use of different filters, the focus of the lens changes and it is necessary to adjust it manually for each filter, because such imaging systems are devoid of automatic focusing. Thus the acquired images may not be perfectly in-focus and have misalignments. In this work we present an automatic solution to acquire a multispectral data cube of aligned images that can be used in document analysis to extract and/or separate information through enhancement techniques. A custom-made motorized autofocus system controlled by software has been installed on a filter-wheel camera, which allows us to acquire in-focus images automatically at each filter changing, and a registration method based on Fourier-Mellin transform aligns these images. A preliminary calibration is performed to set the focusing of each filter with the working distance. As a result, the camera can capture images independently from the contrast of the scene, also in presence of documents with homogeneous texture. The proposed solution reduces considerably the acquisition time and offers a tool to acquire automatically the data cube to be used in further image analysis techniques. The results of experimentations are hereby presented and discussed.  相似文献

The timely diagnosis of acute coronary syndrome (ACS), in particular myocardial infarction (MI), is still one of the most challenging issues in medicine. The introduction into routine laboratory practice of assays for measuring the cardiospecific troponins has dramatically revolutionized the diagnostic approach and the recent development of methods with improved analytical sensibility (i.e., highly sensitivity [HS] assays), has further contributed to improve the negative predictive value of troponin testing but, contextually, has substantially lowered the clinical specificity of these markers. In particular, clinical studies have demonstrated the existence of an exercise-related increase of HS-troponins, with measurable values detectable in up to 94% of athletes undergoing endurance sports. This measurable amount oftroponin in blood would mirror an increased membrane permeability and early troponin release rather than reflecting a clinically threatening myocardial injury. As such, the measurable amount of cardiac troponins as assessed with the novel HS assays requires major clinical focus (i.e., serial measurement of cardiac biomarkers, detailed clinical history-taking, integration with ECG and imaging findings) to prevent misdiagnosis of ACS and/or MI in otherwise healthy persons.  相似文献

I propose a method to synthesize the performance scores for artistic sports such as rhythmic gymnastics, figure skating, synchronized swimming, and diving by taking into account inter-judge variability, while maintaining all the reliable scores. This procedure is based on the assumption that the majority of the scores in each event are reliable and they relate well to those scores that are closest to them. The method consists of putting scores in order and considering clusters of m consecutive scores, where m is the number of judges making up the simple majority. For each cluster, the difference between the highest and the lowest score is calculated. In cases where the minimum difference is positive, the arithmetic mean of those scores that belong to clusters where the difference is minimal is computed. In cases where the minimum difference is zero (i.e. if the majority of judges unanimously assign the same score), then the set of the scores to consider within the mean is extended to those scores that are very near to those of the majority of the judges. A comparison between the actual evaluation procedures and the proposed model is provided.  相似文献

Background:

In vitro hemolysis can be induced by several biological and technical sources, and may be worsened by forced aspiration of blood in vacuum tubes. This study was aimed to compare the probability of hemolysis by drawing blood with a commercial evacuated blood collection tube, and S-Monovette used either in the “vacuum” or “aspiration” mode.

Materials and methods:

The study population consisted in 20 healthy volunteers. A sample was drawn into 4.0 mL BD Vacutainer serum tube from a vein of one upper arm. Two other samples were drawn with a second venipuncture from a vein of the opposite arm, into 4.0 mL S-Monovette serum tubes, by both vacuum an aspiration modes. After separation, serum potassium, lactate dehydrogenase (LD) and hemolysis index (HI) were tested on Beckman Coulter DxC.

Results:

In no case the HI exceed the limit of significant hemolysis. As compared with BD Vacutainer, no significant differences were observed for potassium and LD using S-Monovette with vacuum method. Significant increased values of both parameters were however found in serum collected into BD Vacutainer and S-Monovette by vacuum mode, compared to serum drawn by S-Monovette in aspiration mode. The mean potassium bias was 2.2% versus BD Vacutainer and 2.4% versus S-Monovette in vacuum mode, that of LD was 2.7% versus BD Vacutainer and 2.1% versus S-Monovette in vacuum mode. None of these variations exceeded the allowable total error.

Conclusions:

Although no significant macro-hemolysis was observed with any collection system, the less chance of producing micro-hemolysis by S-Monovette in aspiration mode suggest that this device may be used when a difficult venipuncture combined with the vacuum may increase the probability of spurious hemolysis.  相似文献

In subjectively assessed artistic sports, judges sometimes collude. To avoid this, Federations adopt various strategies in their regulations. However, it is evident that these methods are often not sufficient to overcome collusions completely. The problem is especially difficult in cases where the judging panel consists of sub-commissions that deal with particular aspects of each performance and the scores of each sub-commission are summarized as mean values, which, in turn, are combined into a final mark. Therefore, the formulation of anti-collusion indices would be useful to assess judges and take account of such scores, rejecting the less reliable ones. An anti-collusion index and related average, recently introduced, can be applied in fields other than sports. This article presents some useful considerations for the application of such instruments to the field of sport.  相似文献

In the UK the Research Assessment Exercise (RAE) is being replaced by the Research Excellence Framework (REF); similarly, in Italy the former Research Evaluation Exercise (VTR 2001–2003) has been revised and extended and a new cycle of assessment was activated in Autumn 2011 (VQR 2004–2010). The analysis of the development and the consequences of both British and Italian assessment exercises is carried out on the basis of three theoretical frameworks – new public management (NPM), neo-institutional sociology (NIS), and organisational control theory (OCT). This last framework is crucial because the control systems have an effective sense-making capacity and it also exposes a different awareness about control systems in the British and Italian experiences.  相似文献

This paper focuses on assessing possible levers in the hands of a university’s top management team for inducing or managing change when faced with environmental drives. The topic is discussed after analysing change in a university over a 20-year period. The case study underlines the opportunities and difficulties of linking external environmental drives and change processes, highlighting the lack of a universal path and proposing elements that are worthy of attention. The paper underlines the need for top management teams in universities to pay particular attention to the systemic and balanced aspects of the factors determining change.

This paper illustrates the derivation of a linear parameter varying (LPV) model approximation of a turbocharged Spark-Ignition (SI) automotive engine and its usage in designing a model-based fault detection and isolation (FDI) scheme. The LPV approximation is derived from a detailed nonlinear mathematical model of the engine on the basis of the well known Jacobian approach. The resulting LPV representation is then exploited for synthesizing a bank of LPV-FDI H/H? Luenberger observers. Each observer is in charge of detecting a particular class of fault and is designed for having low sensitivity to all other exogenous inputs so as to allow an effective fault isolation. The adopted FDI scheme is gain-scheduled and exploits a set of engine variables, assumed to be measurable on-line, as a scheduling parameters. The goodness of the LPV approximation of the engine model and the effectiveness of the LPV-FDI architecture are demonstrated by several numerical simulations.  相似文献
