Introduction

Screening programs for colorectal cancer (CRC) are mainly based on a first-line fecal immunochemical test for hemoglobin (FIT). Fecal M2-type pyruvate kinase (M2-PK) has been evaluated in clinical settings showing promising results for early CRC detection. However, the impact of fecal M2-PK assessment on the performance of first-round CRC screening programs is not known. We investigated whether fecal M2-PK alone or in combination with FIT may improve CRC screening efficacy in the general population.

Materials and methods

A total of 1027 asymptomatic subjects (median age 66 [59-74] years; females 504 [49.1%]), identified through the general practitioners’ rosters, were invited for the collection of 2 fecal samples for FIT and M2-PK evaluation. Participants with at least positive one fecal test were referred for colonoscopy. Quality indicators for screening performance were calculated and analyzed using Fisher’s exact test.

Results

Overall, 572 subjects underwent both FIT and M2-PK assessment (participation rate 55.7%): 93 participants showed positive results for at least one test (positivity rate 16.3%). Only 10 patients were positive for both tests. Attendance rate to colonoscopy was 86.0% and a total of 65 adenomas and 7 cancers were detected. Combined use of FIT and fecal M2-PK permitted the identification of 18 more neoplasm (25%) without improving colonoscopy workload, as deduced by the comparable number needed to scope (P = 0.402).

Conclusion

The addition of M2-PK testing to FIT offers the potential to detect additional neoplasms that either do not bleed or only bleed intermittently without reducing participation rate and without increasing endoscopy workload.Key words: biomarker, colorectal cancer, occult blood, M2-type pyruvate kinase, screening  相似文献

Abstract To investigate the effects of ageing on the neural control strategies governing sprint cycling on a friction-loaded cycle-ergometer, 10 older (aged 70-83yr) and 8 young (aged 19-35yr) healthy women completed seven 6-s all-out cycling trials against varying loads. Root mean square (RMS), median frequency and muscle fibre conduction velocity were determined from the vastus lateralis of the dominant limb during each pedal stroke. Peak power was 43% lower in the older group compared to the younger (p??0.05). Although sort has been extensively studied in many research works, it still remains a challenge in particular if we consider the implications of novel processor technologies such as manycores (i.e. GPUs, Cell/BE, multicore, etc.). In this paper, we compare different algorithms for sorting integers on stream multiprocessors and we discuss their viability on large datasets (such as those managed by search engines). In order to fully exploit the potentiality of the underlying architecture, we designed an optimized version of sorting network in the K-model, a novel computational model designed to consider all the important features of many-core architectures. According to K-model, our bitonic sorting network mapping improves the three main aspects of many-core architectures, i.e. the processors exploitation, and the on-chip/off-chip memory bandwidth utilization. Furthermore we are able to attain a space complexity of Θ(1). We experimentally compare our solution with state-of-the-art ones (namely, Quicksort and Radixsort) on GPUs. We also compute the complexity in the K-model for such algorithms. The conducted evaluation highlight that our bitonic sorting network is faster than Quicksort and slightly slower than radix, yet being an in-place solution it consumes less memory than both algorithms.  相似文献

The seismic assessment of art objects requires a multidisciplinary approach and the definition of a specific methodology for evaluating the level of safety and the possible interventions necessary for seismic risk prevention, finding a compromise between safety and conservation. This paper describes a first step in this direction, in which the different aspects of the procedure, from knowledge path of the art object to the structural analysis, were studied in the six Michelangelo's statues located in the “Galleria dei Prigioni” (Slaves’ Gallery) at the Accademia Gallery of Florence. We showed that the general principles and criteria expressed in the “Italian Guidelines for evaluation and mitigation of seismic-risk to cultural heritage”, which are specifically devoted to masonry constructions, could be profitably extended to other artworks, like statues, starting from historical research, geometrical survey and material characterization, up to the developing of specific methods of analysis. This research could be the base for future studies to be performed concerning the prevention of seismic damage of art objects, including both bare minimum interventions and specific devices, such as isolation systems.  相似文献

At school, pupils often cooperate on common projects and must coordinate their different individual actions. However, grades are pervasively used even in cooperative situations, which make the pupils’ differences in achievement and their relative rank salient and may reduce their inclination to work constructively with others. Thus, we hypothesized that grades would elicit disruptive interactions and reduce performance in a cooperative cognitive-motor task necessitating inter-individual coordination of members. In a study with 5th graders, grades (vs. a neutral concept) were primed at the onset of a cooperative group interaction. Results showed that, although pupils were set to work cooperatively, priming grades (vs. neutral concepts) harmed inter-individual coordination by eliciting more negative dominant behaviours among pupils during interactions, which decreased group performance.  相似文献

Previous studies with university students have shown that resource interdependence during cooperative dyadic work on texts produces two different dynamics in student interaction and learning. Working on complementary information produces positive interactions, but a good quality of information transmission is needed to foster student learning. Working on identical information produces a confrontation of viewpoints but also encourages a threatening social comparison of competence, which can be detrimental for learning. The aim of present study is to test the moderating role of a partner’s competence in two peer-learning methods by manipulating a partner’s competence through a confederate. Results indicate that a partner’s competence is beneficial when students work on complementary information while it is detrimental when students work on identical information.  相似文献

In this paper we propose a new event generator, which has strong noise-filtering capabilities, to be used in event-based control systems with a PIDPlus controller. An approximate frequency analysis is performed in order to characterize the event generator system and tuning guidelines are provided for its design parameter. Simulation and experimental results obtained with a laboratory setup demonstrate the effectiveness of the methodology in providing a satisfactory performance related to set-point and load disturbance step responses with a total variation that is significantly reduced with respect to the standard cases.  相似文献

This paper presents the findings of a study undertaken by the United Kingdom Quality Assurance Agency for Higher Education (QAA), with support from the International Network for Quality Assurance Agencies in Higher Education (INQAAHE), aimed at investigating the challenges and limits to cross-border cooperation in the quality assurance of transnational education. The study builds on the outcomes of the recently concluded Erasmus Mundus project Quality Assurance of Cross-Border Higher Education (QACHE) and in particular the QACHE Toolkit developed as part of the project to foster cooperation between quality assurance agencies in quality assuring transnational education. Based on the responses to a survey sent to QAA’s partner agencies in key sending and receiving countries of transnational education, the study extract recommendations to agencies to help them developing viable strategies for inter-agency cooperation, identifying concrete ways in which they might or might not cooperate across borders.  相似文献

The knee is one of the most frequently injured joints of the human body with injuries affecting the general population and the athletic population of many age groups. Dissection procedures for the knee joint typically do not allow unobstructed visualization of the anterior cruciate or posterior cruciate ligaments without sacrificing the collateral ligaments. In many cases, the relationships of the intraarticular structures are lost as dissection systematically removes superficial structures to gain access to deeper structures. The authors present an alternative technique for dissection of the human knee joint that allows maximal visualization of intraarticular structures such as the cruciate ligaments and menisci with minimal disturbance to the tibial and fibular collateral ligaments, thus preserving the relationships of the ligamentous and intraarticular structures.
