目的:研究洛铂联合重组人血管内皮抑素(恩度)治疗肺癌恶性胸腔积液的有效性和安全性。方法:对确诊为肺癌恶性胸腔积液的63例患者进行分析,采用胸腔内注射治疗方法,其中32例注入洛铂和恩度(A组),31例注入顺铂和恩度(B组),比较两组间疗效、生活质量和毒副作用。结果:洛铂和恩度组与顺铂和恩度组在疗效和生活质量改善上比较差异无统计学意义(P〉0.05),在毒副作用方面,洛铂和恩度组白细胞(WBC)减少、贫血、恶心呕吐发生率较顺铂和恩度组低,差异有统计学意义(P〈0.05)。结论:洛铂联合恩度胸腔内注射治疗肺癌恶性胸腔积液疗效与顺铂联合恩度相当,毒副作用较轻。 相似文献

目的总结恶性心包积液置管引流注药化疗的临床护理的经验。方法22例患者采SeLdinger技术心包置管,深静脉导管引流后,注入化疗药物,加强导管护理,注意临床观察。结果本组置管28例次,导管阻塞4例次(14.28%),导管滑脱2例次(7.14%),穿刺点炎症3例次(10.71%),未见导管断裂。结论加强对心包置管的护理是此疗法的关键,适当的护理保证并发症少,患者痛苦少,治疗效果佳。 相似文献

The aim of this study was to assess changes in body morphology, anatomical alignment and prevalence of patellofemoral pain (PFP) in young female dancers along one year of pubertal growth, and to identify the risk factors related to PFP in these young dancers. Both legs of 60 dancers were evaluated during grade 7 and again after 1-year. At each of these points in time, the dancers were interviewed concerning their background, and anthropometric measurements, lower-limb physical examinations and knee ultrasound scans were performed. Morphological parameters changed significantly from baseline to follow-up. PFP was found in 53.3% of the dancers’ knees at baseline. At follow-up, 55.4% of the asymptomatic knees at baseline developed PFP, and only 9.4% of the symptomatic knees at baseline recovered. Lower BMI was identified among dancers who developed PFP during follow-up compared with dancers with no PFP, either at baseline or at follow-up. A positive grinding and positive Patellar Inhibition Test (PIT) were found to be risk factors for PFP at follow-up. A high prevalence of young dancers suffered PFP, from injuries they sustained mostly during the 1-year of dance practice. Parameters predisposing the dancers to PFP should be identified at early stages of dance class. 相似文献

An unembalmed cadaveric preparation for simulating pleural effusion: A pilot study of chest percussion involving medical students

Cadaveric simulations are an effective way to add clinical context to an anatomy course. In this study, unembalmed (fresh) cadavers were uniquely prepared to simulate pleural effusion to teach chest percussion and review thoracic anatomy. Thirty first‐year medical students were assigned to either an intervention (Group A) or control group (Group B). Group A received hands‐on training with the cadaveric simulations. They were instructed on how to palpate bony landmarks for identifying the diaphragm and lobes of the lungs, as well as on how to properly perform chest percussion to detect abnormal fluid in the pleural space. Students in Group B practiced on each other. Students in Group A benefited from the training in several ways. They had more confidence in their percussive technique (A = mean 4.3/5.0, B = 2.9/5.0), ability to count the ribs on an intact body (A = mean 4.0/5.0, B = 3.0/5.0), and ability to identify the lobes of the lungs on an intact body (A = mean 3.8/5.0, B = 2.3/5.0). They also demonstrated a greater ability to locate the diaphragm on an intact body (A = 100%, B = 60%) and detect abnormal pleural fluid (A = 93%, B = 53%) with greater confidence (A = mean 3.7/5.0, B = 2.5/5.0). Finally, the hands‐on training with the unembalmed cadavers created more excitement around learning in Group A compared with Group B. 目的:探讨胸水的腺苷脱氨酶(ADA)测定值对结核性胸膜炎的诊断意义和应用价值。方法:对436例住院的胸腔积液患者分别行结核菌素纯蛋白衍生物(PPD)试验,同时应用酶速率法检测胸水标本中ADA的活性.比较二者的灵敏度及特异度。所有患者试验前均未行抗结核治疗。结果:ADA试验灵敏度(96.1%)高于PPD(90.2%)。二者差异有显著性(P〈0.05)。结论:胸腔积液ADA含量的检测对于早期诊断及治疗结核性胸膜炎具有积极意义。 相似文献

The adjustments and diagnostic significance of polyacrylamide gel electrophoretic (PAGE) profiles of lactate dehydrogenase
isoenzymes (LDH: 1.1.1.27) was evaluated in the sera and pleural fluid of patients with tubercular pyothorax. Sera and pleural
fluid samples were randomly collected from 72 and 18 patients respectively at two different timings; first, when patients
were admitted to the Hospital and second, after an intensive phase of treatment. Sera of 20 healthy individuals served as
control. Our results demonstrate significant differences in sera LDH (sLDH) and pleural fluid LDH (pfLDH) isoenzymes. In patients
the order of LDH isoenzyme in sera and pleural fluid followed: LDH−5>−4>−2>−3>−1 and LDH-5>−4>−3>−2>−1 respectively. The ranking
of activity levels in control was LDH−2>−1>−3>−5>−4. In the second phase of sampling from 31 patients, values of sLDH isoenzymes
showed recovery and resembled profiles of controls. Therefore, the sLDH zymograms of patients can be used as the prognostic
marker, since they tend to reach the normal level during recovery signifying the effect of chemotherapy in hospitalized patients.
Moreover, according to the present findings on LDH-PAGE profiles, the levels of LDH-5 and-4 rise in pyothorax patients significantly
(P<0.05). This elevation along with the rise in total LDH activity may, therefore, be used in the diagnosis and monitoring
of tubercular pyothorax. 相似文献

The pathological accumulation of serous fluids in the pleural, peritoneal and pericardial space occurs in a variety of conditions. Since patient management depends on right and timely diagnosis, biochemical analysis of extravascular body fluids is considered a valuable tool in the patient management process.The biochemical evaluation of serous fluids includes the determination of gross appearance, differentiation of transudative from exudative effusions and additional specific biochemical testing to assess the effusion etiology. This article summarized data from the most relevant literature concerning practice with special emphasis on usefulness of biochemical tests used for the investigation of pleural, peritoneal and pericardial effusions. Additionally, preanalytical issues concerning serous fluid analysis were addressed and recommendations concerning acceptable analytical practice in serous fluid analysis were presented. 相似文献

Tuberculosis has, in a short span of time, become a major health problem in the third world or developing countries like India.
In view of this, a retrospective study was conducted to study Adenosine deaminase activity in serum and pleural fluid in patients
affected with Pulmonary Tuberculosis and other common non-tubercular chronic respiratory diseases. The study was carried out
on 100 patients suffering from various pulmonary disorders, between January 2002 and August 2002. Thirty-five normal healthy
individuals were included as control subjects. ADA estimation was done by sensitive colorimetric method. The study revealed
that the serum ADA activity was higher in patients of tuberculous pulmonary and pleural diseases and non-tuberculous pulmonary
diseases than in control subjects. The mean serum ADA activity in the patients’ group was 35.5±6.93 u/l as compared to 16.20±2.85
u/l in control group, showing a highly significant (P≪0.001) difference. ADA activity was highest in tuberculous pleuropulmonary
diseases. The pleural fluid ADA activity was higher in pyogenic pleural effusion than in tuberculous pleural effusion. 相似文献
