| 摘 要： | 目的：为了了解生命维持技术应用中存在的医学、经济、伦理、社会等问题,在全省各地医院完成了关于生命维持技术应用状况的临床调查,时间从2000年至2003年,共完成有效例数199例.方法：用自编生命维持技术临床调查表对应用生命维持技术的患者及其家属、主治医护人员按问卷项目进行访谈,用spss10.0进行统计学分析.结果：生命维持技术的应用在挽救病人的生命和维持重要脏器功能衰竭病人的生存以及延迟死亡方面发挥了积极作用,但总死亡率仍较高.存在一定比例的对脑死亡或长期昏迷患者的维持.多数患者愿意使用生命维持技术并期待更好的技术出现.患者的家属、单位、主治医护人员对该患者应用生命维持技术从技术、伦理、经济等方面予以支持.生命维持技术医疗费用的来源以家庭支付和基本医疗保险为主.不同医疗费用来源、职业、经济状况与维持效果无相关性.因经济困难提前终止生命维持、放弃治疗者出现较高比例.在生命维持技术应用的价值判断和伦理选择问题上,应坚持患者自主原则、人道主义原则、患者利益原则,正确理解生命至上论与生命价值论、生命质量论,反对社会公益论.

| Abstract: | Aims: In order to find out medical,economical,social and ethical issues existing in the process of the application of Life-Sustaining Technique,the author organized the graduate students and the students on-the-job training a survey on the application of Life-Sustaining Technique in 2000 and 2003.One hundred and ninety-nine cased were effective.Methods: To investigate the patients,their relation and attending doctors with self-made Life-Sustaining Technique clinical questionnaire and visit them according to items in the questionnaire.Results: Life-Sustaining Technique had a positive effect on saving dying patients' life,supporting the patients who had prostrate hearts and reserving the life.Most of the patients would choose this technique and expected better development.The patients' relation,unit,attending doctors provided support for the patients in the aspect of technique,ethic and economy.The fees of this technique came from the patients' family and medical insurance.The source of the fees,patients' occupation and economical conditions had no relationship with the effect of the technique.The death rate were still very high.There were a large proportion of patients gave up the technique and medical treatment due to the poverty.In terms of value estimation and ethic selection in the process of Life-Sustaining Technique,the principles of self-determination by the patient,interest for the patient and humanism should be insisted on.Views on supremacy of life,value of life and quality of life should be correctedly understood and views of public interest should be strongly against. |
