| 摘 要： | 集体经济时期,赤脚医生是农村合作医疗制度的核心,他们以尽可能低的成本保障所在村农民的基本健康,他们归属感强烈,这一社会事实表达了村医个体与组织集体之间的一致性;集体经济解体后,村医自负盈亏自我发展,他们利用"结构洞"优势追求个人经济利益,其行动逻辑与国家市场改革导向一致;新农合实施后,除定点卫生室以外的农村绝大部分村医的医疗服务被排斥在制度设计范围之外,村医的功能被重新定位,他们被要求只提供卫生服务。不同时期的农合制度改变村医行为的事实丰富了中国经验。
