胸腹内脏器病变的CT/US导引经皮细针穿刺活检的临床研究

摘    要:目的应用和比较不同穿刺针、不同导引技术对穿刺活检结果的影响。材料和方法90例中男性63例,女性27例,平均年龄54岁(23-78岁),胸部66例,腹部24例;按穿刺针不同分DFBN(dispos able fanseen biopsy needle)和ADGSTN(automatic disposable guillotine soft tissue needle)两组;比较两组的诊断正确率、并发症发生情况。结果所有病例均穿刺成功并获得细胞学和/或组织学材料,细胞学检查正确者9/25例(36%),组织学检查正确者66/91例(72.53%),DFBN和ADGSTN两种穿刺针在标本的大小和完整性、病理学结果、诊断正确率(55.81%/91.30%)有差异(P<0.01),穿刺活检在肿瘤患者中的诊断正确率为78.95%,发生并发症8例(8.89%),其中气胸2例,咯血或痰中带血6例。结论CT/US 导引经皮细针穿刺活检是一项先进的影像学与病理学活检相结合的技术,对于胸腹内不明原因的占位性病变,尤其是怀疑肿瘤者是一项简便易行、较安全的诊断手段。ADGSTN较DFBN材料满意且诊断正确率高。

Abstract:This article was a study on 90 cases of intra-thoracic and intra-abdominal lesions in application of CT/US guided percutanous fine needle biopsy(PFNB) with different fine needles and guided techniques. The results showed that the CT/US guided fine-needle puncture of the lesion was successful and cytologic and/or histologic specimens were obtained. The accuracy rate was 36% in cytology and 72.53% in histology. There was significant difference in diagnostic accuracy rate between DFBN(disposable franseen biopsy needle,55.81%)and ADGSTN(automatic disposable guillotine soft tissue needle,91.30%) (P<0.01). The diagnostic accuracy rate in tumor patients was 78.95%. The technique was a simple, safe method for identifying space-occupying lesions in the intra-thoracic and intra-abdominal cavity, especially for the suspected malignant tumor cases.
