| 摘 要： | 基于中国五个经济发达城市新型农村合作医疗制度实施状况的调查发现,这些城市的农民参合率逐年上升甚至接近全民参合。合作医疗资金完全依靠本市财政以及农民个人的缴纳。同时,参合主体在参加医疗保障制度种类选择上具有较强灵活性。但是,也存在若干急待解决的问题,例如,较高参合率依赖于非正式制度如宣传的介入,从而增加了筹资成本;合作医疗资金筹集以及补偿缺乏科学测算。另外,村卫生室尚未纳入到合作医疗定点医疗机构当中去。这些问题将影响城市新型农村合作医疗制度的持续发展。有鉴于此,经济发达城市应当在坚持自愿性基础上制定相应的法律法规,逐步向强制性过渡,尽快建立起与社会医疗保险相互衔接的新型农村合作医疗资金筹集与使用精算模型,努力将农村卫生室纳入到合作医疗服务体系当中去,充分发挥村卫生室在疾病防治中的基础性作用,真正实现新型农村合作医疗制度的持续发展。
