| 摘 要： | 目的探讨胸腰椎骨折脱位的手术治疗方法.方法采用Dick技术对46例胸腰椎骨折脱位的患者,经椎弓根对骨折脱位椎体施行国产Dick钉复位内固定.其中下胸椎7例、腰椎34例、单纯脊柱滑脱椎管狭窄5例.结果随访2～10年,平均6年11个月,固定满意39例,占85%;失误7例,占15%,其中5例再次手术固定满意;术后无一例并发症发生,总优良率为95%.结论Dick技术是治疗脊柱骨折脱位的较好方法,主要适用于下胸段、胸腰段及腰段脊椎损伤合并脊髓损伤,手术成功关键为Schang椎弓根螺丝钉的进钉部位、角度和深度.

| Abstract: | Objective: To explore the surgical treatment method of thoracolumbar fractures and dislocations. Methods: 46 patients with thoracolumbar fractures and dislocations were enrolled treated with Dick's technique. Among them, there were 7 cases in the lower thoracic vertebrae, 34 cases in the lumbar vertebrae and 5 cases in lumbar spinal stenosis led by single vertebrae dislocations. With Dick's screw(made in China), the vertebral bodies with fracture and dislocation were operated by and internal fixation through pedicle of vertebral arch. Results The patients were followed up for 2 to 10 years, 6 years and 11 months in arerage. Adopting the aforementioned method, 39(85% ) cases reached an excellent or good effect, and 7 cases (15% )were not cured. 5 cases of the uncured were operated again and cured finauy. No complication occured in all patients. The total cure rate was 95%. Conclusion Dick's technique is a better method in treating the thoracolumbar fractures and dislocations. It mainly applies to the fractures and dislocations' s position in the lower thoracic vertebraes and lumbar vertebraes, which complicated with spinal cord damage at the same time. The key to the success of the operation is the position, angle and depth of the Schang's pedicle screw at which is penetrated. |
