| 摘 要： | 目的 :了解肺结节病的临床表现的不典行性和确诊方法 ,从而加深对肺结节病的认识。方法 :回顾分析了 64例经病理证实的肺结节病患者的全部资料。结果 :根据X线征象诊断或考虑为肺结节病的确诊率为 59.3% ,经支气管镜粘膜活检的确诊率为 67.5 % ,经支气管镜肺活检(TBLB )的确诊为率 62 .5 % ,PPD试验阴性者 82 .8% ,常规行SACE检查增高者为73 .4%。结论 :因肺结节病临床表现具多样性和不典型性 ,故易导致漏诊和误诊。其诊断依赖病史、临床表现、X线、支气管镜、肺活检等多种方法 ,病理诊断是主要的确诊依据。并提倡对可疑的肺结节病患者应尽早行支气管镜粘膜活检或经TBLB检查 ,以期早日明确诊断

| Abstract: | Objective To study the clinical symptom and the method of diagnosis FOR lung sarcoidosis ,so as to enhance the knowledge of lung sarcoidosis . Met hod : 64 cases of lung sarcoidosis confirmed by pathology were reviwed and anali zed. Results: the diagnosis rate of lung sarcoidosis is 59. 3 % depending on x-ray , the diagnosis rate is 67.5 % depending on transbronchial mucosa bio psy; the TBLB rate is 62.5 % depending on transbronchial lung biopsy. The negati ve rate of PPD test is 82.8 % , the positive rate of SACE test is 73.4 %. Conclusion: the clinical symptom of lung sarcoidosis is varia ble. not typical . As a result, this will cause an erroneous diagnosis or get the doctor fallin g in the examination .The diagnosis of the type of disease depends on medical records ,clinical symptom x-ray .transbronchial lung biopsy and so on .Patholo gical diagnosis is the basical of examine. Transbronchial mucosa biopsy and lu ng biopsy are very important to those who are suspected having lung sarcoidosis ,because they can be diagnosed early in this way. |
