Managed health care deliberations in Oregon point to a "civic bioethics" model to resolve allocation issues through publicly articulated community values and priorities that influence legislative action. Communication practices in the Oregon experiment provide data to develop an evaluative framework to assess public moral deliberation in value-laden contexts such as health care distribution. The Oregon experience also points to dialogical virtues to heighten citizens' ethical sensibilities and political will, discursive tools to inform ethical communication practices in a civic bioethics model.  相似文献

SUMMARY

Oregon voters are heavy users of the initiative and referendum, and they were pioneers of this form of democracy. This means that there is a good deal of information to be found about past initiatives and referendums, and the government of the State of Oregon and local civic groups have responded by making much of this information available online. Participatory, arts-based methods generate rich data with which researchers can explore information behavior in context, and may be particularly apt when engaging with youth or participants with low literacy levels. Information world mapping (IWM) is an innovative and interactive drawing-based interview technique for data elicitation. Initially developed for use in a study of young parents’ health information practices, IWM guides participants in depicting their personal social information worlds, including items, places, and relationships. Maps are then used to facilitate critical incident elicitation of participants' stories about, and interpretations of, their information practices. Within the young parent study, three styles of map were commonly seen: the directional map, the mind map, and the symbolic map. Use of IWM requires time and ethical care, but the method enables researchers to center participants’ own perspectives on information practices, triangulate data obtained via more traditional methods, and enrich understanding of social information worlds.  相似文献

This article provides a reflection on the outcomes of an international collaboration between health librarians and academics at York St John University and Pacific University Oregon. In particular, it describes how a month‐long visiting professorship from an academic with a clinical librarian background at the Faculty of Health and Life Sciences helped to develop and inform teaching practice in the areas of information literacy and evidence‐based health practice on health programmes at Pacific University. Perspectives are offered from both institutions on the rich exchange of knowledge and practice that took place during the visit and the ongoing impact it has had on teaching practices. H. Almost 15 years after it started, civic journalism is waning. Some say that its practices have been integrated into the routines of news making without the label attached. Others say that it is simply dying. This study seeks to define the legacy of civic journalism by investigating the news practices in Savannah Morning News, a newspaper in Georgia, USA. Ethnographic observation and interviews found that the ideas of civic journalism were instituted in the newspaper through its presentation and the routines of discovering community news. However, it was less obvious in the discovery and gathering of news about larger events and issues. The role of the news organization in convening the public for problem solving has continued, but the role of championing particular solutions was not observed.  相似文献

Purpose:

This lecture discusses a philosophy of educating health information professionals in a rapidly changing health care and information environment.

Discussion:

Education for health information professionals must be based upon a solid foundation of the changing paradigms and trends in health care and health information, as well as technological advances, to produce a well-prepared information workforce to meet the demands of health-related environments. Educational programs should begin with the core principles of library and information sciences and expand in interdisciplinary collaborations. A model of the health care environment is presented to serve as a framework for developing educational programs for health information professionals.

Conclusion:

Interdisciplinary and collaborative relationships—which merge health care, library and information sciences, and other information-related disciplines—should form the basis of education for health information professionals.

Quality of health information on the Internet has been a concern since health information first began appearing on the Web. Evidence-based medicine tools, traditionally intended for physicians, may benefit consumers as they participate in making health care desisions. This article describes a rationale for Cochrane reviews as an evidence-based medicine tool for consumers. The Cochrane Collaboration, a global force for systematic literature reviews, has strict procedures for developing literature reviews. Criteria for Cochrane reviews are compared with critical evaluation skills commonly taught to consumers regarding the use of Websites. The Cochrane Collaboration''s Consumer Network has established a separate Website, with review synopses written for an audience of consumers. Suggestions for further research into consumer use of the Cochrane Library and consumer involvement with the Cochrane Collaboration are discussed.People who access health information are likely to play an active role in their health care [1]. Access to health information empowers consumers by enabling them to (1) make more knowledgeable choices in self-care, (2) more intelligently discuss medical conditions and treatments with their doctors, (3) educate themselves regarding good health practices, and (4) learn about the health care system. If access to information is to help consumers achieve better health, quality of accessible information must be considered by information producers and consumers. This paper identifies a rationale for evidence-based medicine for consumers, introduces the quality-control work of the Cochrane Collaboration, and explores consumer involvement in the Cochrane Library, relative to critical evaluation models.  相似文献

Objective:

The investigation provides recommendations for establishing institutional collection guidelines and policies that protect the integrity of the historical record, while upholding the privacy and confidentiality of those who are protected by Health Insurance Portability and Accountability Act (HIPAA) or professional ethical standards.

Methods:

The authors completed a systematic historical investigation of the concepts of collection integrity, privacy, and confidentiality in the formal and informal legal and professional ethics literature and applied these standards to create best practices for institutional policies in these areas.

Results:

Through an in-depth examination of the historical concepts of privacy and confidentiality in the legal and professional ethics literature, the authors were able to create recommendations that would allow institutions to provide access to important, yet sensitive, materials, while complying with the standards set by HIPAA regulations and professional ethical expectations.

Conclusion:

With thoughtful planning, it is possible to balance the integrity of and access to the historical record of sensitive documents, while supporting the privacy protections of HIPAA and professional ethical standards. Although it is theorized that collection development polices of institutions have changed due to HIPAA legislation, additional research is suggested to see how various legal interpretations have affected the integrity of the historical record in actuality.

Between 1987 and 2013, HBO produced or distributed over twenty HIV/AIDS programs. These films trace a cultural shift from an early focus on AIDS as a public health issue to be dealt with through individual “safe-sex” practices and ethical citizenship to a later focus on AIDS as a global pandemic where the explicit strategy becomes a reliance on non-state actors to combat AIDS. This article argues that HBO's HIV/AIDS films are embedded within a cultural approach to AIDS that relies on governmental logics and neoliberal solutions – not direct action, but directing action.  相似文献

Objective

This study investigated responsibilities, skill sets, degrees, and certifications required of health care navigators in order to identify areas of potential overlap with health sciences librarianship.

Method

The authors conducted a content analysis of health care navigator position announcements and developed and assigned forty-eight category terms to represent the sample''s responsibilities and skill sets.

Results

Coordination of patient care and a bachelor''s degree were the most common responsibility and degree requirements, respectively. Results also suggest that managing and providing health information resources is an area of overlap between health care navigators and health sciences librarians, and that librarians are well suited to serve on navigation teams.

Conclusion

Such overlap may provide an avenue for collaboration between navigators and health sciences librarians.  相似文献

While the study of the ethical use of new technologies in journalistic work is imperative given the widespread use of such technology, such studies are few and far between, particularly for the developing world. This paper provides results of an exploratory, qualitative study of Indian journalists’ views about the ethical use of new computer and Internet-based technologies for news gathering and reporting in India. New digital technology was widely accessible to the respondents, but not all journalists were given the tools by their employers. Opinions about ethical news practices using new technology were mixed and revealed a few grey areas. English language national newspapers tended to indicate that their standards were strict and that ethical violations, exacerbated by new technology, occurred mostly in vernacular newspapers. On the whole, respondents had not heard often of other organizations or individuals committing unethical practices such as plagiarism and lack of attribution using new technology. Still, some beliefs indicated uncertainty about ethical practice or breach of ethical principles. The findings of this study have implications for journalism education and on-the-job training of Indian journalists, as well as for formulating ethical codes of journalism, particularly with regard to new media. Indian codes focus more on the roles journalists should play in society and do not provide very specific tactical guidance for everyday news gathering. Coupled with lack of training, this creates a situation where perceptions about journalistic ethics are sometimes vague and variant.  相似文献

Objectives:

This research studied hospital administrators'' and hospital-based health care providers'' (collectively, the target group) perceived value of consumer health information resources and of librarians'' roles in promoting health information literacy in their institutions.

Methods:

A web-based needs survey was developed and administered to hospital administrators and health care providers. Multiple health information literacy curricula were developed. One was pilot-tested by nine hospital libraries in the United States and Canada. Quantitative and qualitative methods were used to evaluate the curriculum and its impact on the target group.

Results:

A majority of survey respondents believed that providing consumer health information resources was critically important to fulfilling their institutions'' missions and that their hospitals could improve health information literacy by increasing awareness of its impact on patient care and by training staff to become more knowledgeable about health literacy barriers. The study showed that a librarian-taught health information literacy curriculum did raise awareness about the issue among the target group and increased both the use of National Library of Medicine consumer health resources and referrals to librarians for health information literacy support.

Conclusions:

It is hoped that many hospital administrators and health care providers will take the health information literacy curricula and recognize that librarians can educate about the topic and that providers will use related consumer health services and resources.

Between the years 1993 and 2000, the W. K. Kellogg Foundation sponsored the Comprehensive Community Health Models (CCHMs) Initiative in three Michigan counties. CCHMs was comprised of three closely related community initiatives carried out in the midst of a failed national health care reform effort and the continued penetration of managed care arrangements into many health care systems. This experimental initiative set out to test the hypothesis that traditional healthcare system animosities and exclusionary practices could be overcome by stakeholder participation in an ongoing, structured, collaborative dialogue about improving access to health services. In the process of collecting data through surveys, interviews, content analysis, and observation, we were struck by the occurrence of several overarching tensions that we perceive to exist in our data. The present article elucidates five such tensions and suggests how third parties such as communication researchers, evaluators, and practitioners can facilitate community health improvement initiatives and better their own data interpretation by acknowledging and understanding these tensions.  相似文献

Objective:

Based on the authors'' shared interest in the interprofessional challenges surrounding health information management, this study explores the degree to which librarians, informatics professionals, and core health professionals in medicine, nursing, and public health share common ethical behavior norms grounded in moral principles.

Methods:

Using the “Principlism” framework from a widely cited textbook of biomedical ethics, the authors analyze the statements in the ethical codes for associations of librarians (Medical Library Association [MLA], American Library Association, and Special Libraries Association), informatics professionals (American Medical Informatics Association [AMIA] and American Health Information Management Association), and core health professionals (American Medical Association, American Nurses Association, and American Public Health Association). This analysis focuses on whether and how the statements in these eight codes specify core moral norms (Autonomy, Beneficence, Non-Maleficence, and Justice), core behavioral norms (Veracity, Privacy, Confidentiality, and Fidelity), and other norms that are empirically derived from the code statements.

Results:

These eight ethical codes share a large number of common behavioral norms based most frequently on the principle of Beneficence, then on Autonomy and Justice, but rarely on Non-Maleficence. The MLA and AMIA codes share the largest number of common behavioral norms, and these two associations also share many norms with the other six associations.

Implications:

The shared core of behavioral norms among these professions, all grounded in core moral principles, point to many opportunities for building effective interprofessional communication and collaboration regarding the development, management, and use of health information resources and technologies.  相似文献
