This article describes an epochal review of research literature on practice-oriented digital health ecosystems that have emerged during the period between 1998 and 2018. Practice-orientation refers to efforts that have resulted in positive impact and transformations in the delivery of healthcare. The review is epochal in the sense that there have been 3 major stages of innovation and diffusion in digital health that are identified. The year 1998 was deemed a significant starting point for the literature search because it marked the introduction of the term e-health. The main objective of this review is to track the evolution of digital health and the conspicuous developments in the field - crucial steps to get a grasp of the key issues involving the delivery of healthcare. The article includes a general discussion of these various key issues and sensitizing concepts. The sense-making thus shed some light on the areas of enquiry, which are central to identifying the direction in which the practice of healthcare is headed. These areas of enquiry often referred to as ‘substantive areas’ in qualitative research terminology, were identified as ‘electronic medical records’, ‘health cloud’, ‘data analytics’, ‘Internet-enabled devices’, and other ‘emerging information technologies’. The article concludes with a synthesis of key inferences drawn from the epochal review. The digital disruption of healthcare has created necessary conditions for its offering as a global, interoperable service.  相似文献

中国医疗卫生领域的不平等现象一直是被关注的热点,但相关研究方法局限于从收入、支出以及个人特征角度来考察,目前尚无从机会获得角度对中国医疗保健服务的益贫性进行测度的相关研究.本文利用中国健康与营养调查(CHNS)数据,基于社会机会函数(SOF)构造机会指数(OI)和机会益贫指数(PIO)的方法测度中国卫生保健和医疗服务的益贫性及其动态变化.结果显示,2000 - 2009年中国卫生保健服务不具有益贫性,质量较高的保健机构其服务的益贫性更差;2000年中国总体医疗服务机会基本均等是两个不平衡因素耦合的结果:较贫困病人较多享用了低质量医疗服务,较富有病人较多享用了高质量医疗服务;2009年总体医疗服务的益贫性主要来自村诊所和乡医院等较低质量医疗机构服务的益贫性,县医院和市医院等较高质量的医疗服务益贫性仍然很差.  相似文献

The computerized healthcare information system has undergone tremendous advancements in the previous two decades. Medical institutions are paying further attention to the replacement of traditional approaches that can no longer handle the increasing amount of patient data. In recent years, the healthcare information system based on big data has been growing rapidly and is being adapted to medical information to derive important health trends and support timely preventive care. This research aims to evaluate organization-driven barriers in implementing a healthcare information system based on big data. It adopts the analytic network process approach to determine the aspect weight and applies VlseKriterijumska Optimizacija I Kzompromisno Resenje (VIKOR) to conclude a highly appropriate strategy for overcoming such barriers. The proposed model can provide hospital managers with forecasts and implications that facilitate the withdrawal of organizational barriers when adopting the healthcare information system based on big data into their healthcare service system. Results can provide benefits for increasing the effectiveness and quality of the healthcare information system based on big data in the healthcare industry. Therefore, by understanding the sequence of the importance of resistance factors, managers can formulate efficient strategies to solve problems with appropriate priorities.  相似文献

Members of the lay public are turning increasingly to the internet to answer health-related questions. Some authors suggest that the widespread availability of online health information has dislodged medical knowledge from its traditional institutional base and enabled a growing role for alternative or previously unrecognized health perspectives and ‘lay health expertise’. Others have argued, however, that the organization of information retrieved from influential search engines, particularly Google, has merely intensified mainstream perspectives because of the growing consolidation of the internet with traditional, commercial media sources. In this paper we describe an analysis of ‘first page’ results retrieved through Google searches about several common health concerns, each of which has been the subject of controversy as a result of uncertain aetiology, diagnoses, outcomes and/or contested approaches to treatment. Our findings suggest that the online search tactics used by most lay health information seekers produce sources of information that, for the most part, reflect mainstream biomedical discourses, often linked to commercial interests, rather than a plurality of voices that offer a variety of perspectives and resources. We discuss the implications for health-interested internet searchers who fail to look beyond the ‘first page’.  相似文献

Digital Assistants are overgrowing in the mobile application industry and are now implemented in various commercial devices. So far, their use in the health domain is limited and often narrowed to remote monitoring of specific patient pathology. The main contribution of this paper is HELENA, a conversational agent endowed with healthcare knowledge that supports users in managing their lifelong health user model (LHUM) by providing simple services.The proposed platform has been evaluated in a user study involving 160 participants who gave feedback on the digital assistant and the profile conceptualization. The results showed that LHUM is comprehensive and inclusive (76.25% positive feedback), and the users appreciated that all personal health data were available in one single profile (83.75% positive feedback). Furthermore, the user interaction was pleasant, functional, and efficient. Also, measurement management, data sharing, and food diary management have been broadly appreciated. The outcomes of our investigation suggest that adopting a lifelong comprehensive user profile by digital assistants is a promising research direction.  相似文献

目的:通过对移动医疗App的调查,探讨公众健康信息需求。方法:采用网络调查法获取数据,利用Excel和SPSS软件进行数据处理。结果:医药信息数据库、教育工具、数据跟踪类App数量较多,分别占12%、13%和14%。其中养生保健(11%)、求医挂号(11%)、健身美容类(8%)的App也较多。结论:用户比较关注健康促进,养生保健、求医问药类的健康信息以及可以容易完成的健康跟踪服务,但用户也需要谨慎地选择移动医疗App。  相似文献

Wearable devices introduce many new capabilities to the delivery of healthcare. But wearables also pose grave privacy risks. Furthermore, information overload gets in the way of informed consent by the patient. To better protect American patients in an increasingly digital world, the U.S. Congress passed the Health Insurance Portability and Accountability Act of 1996 (HIPAA). This article examines the adequacy of HIPAA vis-à-vis issues raised by wearable technologies in the Internet of Things environment and identifies policy gaps and factors that drive health data exposure. It presents a 2 × 2 Partnership-Identity Exposure Matrix, illustrates implications in four different contexts, and provides recommendations for improving privacy protection.  相似文献
