Inflammation and haemostasis are interrelated pathophysiologic processes that considerably affect each other. In this bidirectional relationship, inflammation leads to activation of the haemostatic system that in turn also considerably influences inflammatory activity. Such, the haemostatic system acts in concert with the inflammatory cascade creating an inflammation-haemostasis cycle in which each activated process promotes the other and the two systems function in a positive feedback loop. The extensive crosstalk between immune and haemostatic systems occurs at level of all components of the haemostatic system including vascular endothelial cells, platelets, plasma coagulation cascade, physiologic anticoagulants and fibrinolytic activity. During inflammatory response, inflammatory mediators, in particular proinflammatory cytokines, play a central role in the effects on haemostatic system by triggering its disturbance in a number of mechanisms including endothelial cell dysfunction, increased platelet reactivity, activation of the plasma coagulation cascade, impaired function of physiologic anticoagulants and suppressed fibrinolytic activity. The two examples of pathophysiologic processes in which the tight interdependent relationship between inflammation and haemostasis considerably contribute to the pathogenesis and/or progression of disease are systemic inflammatory response to infection or sepsis and acute arterial thrombosis as a consequence of ruptured atherosclerotic plaque. Close links between inflammation and haemostasis help explain the prothrombotic tendency in these two clinical conditions in which inflammation shifts the haemostatic activity towards procoagulant state by the ability of proinflammatory mediators to activate coagulation system and to inhibit anticoagulant and fibrinolytic activities. This review summarizes the current knowledge of the complex interactions in the bidirectional relationship between inflammation and haemostasis.  相似文献

Study on the action mechanism of inflammatory mediators generated by the severe acute pancreatitis(SAP) in mul-tiple organ injury is a hotspot in the surgical field.In clinical practice,the main complicated organ dysfunctions are shock,res-piratory failure,renal failure,encephalopathy,with the rate of hepatic diseases being closely next to them.The hepatic injury caused by SAP cannot only aggravate the state of pancreatitis,but also develop into hepatic failure and cause patient death.Its complicated pathogenic mechanism is an obstacle in clinical treatment.Among many pathogenic factors,the changes of vasoac-tive substances,participation of inflammatory mediators as well as OFR(oxygen free radical),endotoxin,etc.may play important roles in its progression.  相似文献

ABSTRACT

This article examines the participation of ‘third-sector’ organisations in public education in England. These organisations act as a cross-sectoral policy network made up of new kinds of policy experts: mediators and brokers with entrepreneurial careers in ideas. They have sought to make education reform thinkable, intelligible and practicable in terms of a computational discourse consisting of code, networks, interactivity and feedback, and related ideas of decentralisation, open methods and personalisation. What characterises this style of thinking is an ‘anti-political’ preoccupation with computer-coded systems and the idea of networks as a model for new political and educational forms.  相似文献

In this article we introduce a research strategy that involves the making of visual maps by individuals in response to their interactions with artworks. The maps record the meaning‐making processes involved in the encounters and provide us with permanent records of otherwise ephemeral experiences. The case study presented here provides data for comparisons between three visits each to two artworks exhibited at the Calouste Gulbenkian Modern Art Centre in Lisbon. We conclude that our meaning‐making strategy is important for its own heuristic research value in both formal and non‐formal educational contexts, as well as providing an instrument for the training of teachers and museum educators.  相似文献

Acute pancreatitis (AP) is a common acute abdomen in clinic with a rapid onset and dangerous pathogenetic condition. AP can cause an injury of intestinal mucosa barrier, leading to translocation of bacteria or endotoxin through multiple routes, bacterial translocation (BT), gutorigin endotoxaemia, and secondary infection of pancreatic tissue, and then cause systemic in- flammatory response syndrome (SIRS) or multiple organ dysfunction syndrome (MODS), which are important factors influencing AP’s severity and mortality. Meanwhile, the injury of intestinal mucosa barrier plays a key role in AP’s process. Therefore, it is clinically important to study the relationship between the injury of intestinal mucosa barrier and AP. In addition, many factors such as microcirculation disturbance, ischemical reperfusion injury, excessive release of inflammatory mediators and apoptosis may also play important roles in the damage of intestinal mucosa barrier. We investigate a method to estimate the combined effect of multiple continuous/ordinal mediators on a binary outcome: (a) fit a structural equation model with probit link for the outcome and identity/probit link for continuous/ordinal mediators, (b) predict potential outcome probabilities, and (c) compute natural direct and indirect effects. Step 2 involves rescaling the latent continuous variable underlying the outcome to address residual mediator variance and covariance. We evaluate the estimation of risk-difference- and risk-ratio-based effects (RDs, RRs) using the maximum likelihood (ML), mean-and-variance-adjusted weighted least squares (WLSMV) and Bayes estimators in Mplus. Across most variations in path-coefficient and mediator-residual-correlation signs and strengths, and confounding situations investigated, the method performs well with all estimators, but favors ML/WLSMV for RDs with continuous mediators, and Bayes for RRs with ordinal mediators. Bayes outperforms ML/WLSMV regardless of mediator type when estimating RRs with small potential outcome probabilities and in two other special cases. An adolescent alcohol prevention study is used for illustration.  相似文献
