Health policy experts long have argued that a combination of budgetary constraints, lack of legislative expertise, and parochial attitudes make it unlikely that the U.S. states will contribute significantly to health care reform. This is particularly true of states like Texas, which ranks last or near-to-last in virtually every measure of public health, and is dominated by conservative politics and libertarian, free market ideology. Yet, in 1997 the Texas legislature enacted the first Patient's Bill of Rights. This article examines the political and rhetorical processes that led to successful reform, and discusses the implications that success in "the least likely place" holds for the making of health care policy.  相似文献

Managed care reform, commonly referred to as "patients' rights" legislation, has become a hot topic. The SMILE project represented a partnership among the University of Texas Health Science Center at San Antonio Libraries, the Gateway Clinic in Laredo, and the San Antonio Metropolitan Health District. The project focused on improving dental practitioners' access to reliable information resources and integrating the best evidence into public health dental practice. Through its training program, SMILE cultivated a set of "power information users" among the dentists, dental hygienists, and community health workers (promotores) who provided public health preventive care and oral health education. The dental public health practitioners gained information literacy skills and increased their knowledge about reliable sites such as blogs, PubMed?, and MedlinePlus?. This project fostered opportunities for expanded partnerships with public health personnel.  相似文献

This column will describe how a library instruction program is integrated into the occupational therapy, respiratory care, and physical therapy curricula at the University of Texas Health Science Center at San Antonio. Trillions of dollars are spent each year on health care. The U.S. Department of Health and Human Services keeps track of a variety of health care indicators across the country, resulting in a large geospatially multivariate data set. Current visualization tools for such data sets make it difficult to make multivariate comparisons and show the geographic distribution of the selected variables at the same time. Community Health Map is a web application that enables users to visualize health care data in multivariate space as well as geospatially. It is designed to aid exploration of this huge data repository and deliver deep insights for policy makers, journalists, consumer groups, and academic researchers. Users can visualize the geospatial distribution of a given variable on an interactive map, and compare two or more variables using charts and tables. By employing dynamic query filters, visualizations can be narrowed down to specific ranges and regions. Our presentation to policy makers and pilot usability evaluation suggest that the Community Health Map provides a comprehensible and powerful interface for policy makers to visualize health care quality, public health outcomes, and access to care in an effort to help them to make informed decisions about improving health care.  相似文献

PURPOSE: The purpose of the Camp For All Connection project is to facilitate access to electronic health information resources at the Camp For All facility. SETTING/PARTICIPANTS/RESOURCES: Camp For All is a barrier-free camp working in partnership with organizations to enrich the lives of children and adults with chronic illnesses and disabilities and their families by providing camping and retreat experiences. The camp facility is located on 206 acres in Burton, Texas. The project partners are Texas Woman's University, Houston Academy of Medicine-Texas Medical Center Library, and Camp For All. BRIEF DESCRIPTION: The Camp For All Connection project placed Internet-connected workstations at the camp's health center in the main lodge and provided training in the use of electronic health information resources. A train-the-trainer approach was used to provide training to Camp For All staff. RESULTS/OUTCOME: Project workstations are being used by health care providers and camp staff for communication purposes and to make better informed health care decisions for Camp For All campers. EVALUATION METHOD: A post-training evaluation was administered at the end of the train-the-trainer session. In addition, a series of site visits and interviews was conducted with camp staff members involved in the project. The site visits and interviews allowed for ongoing dialog between project staff and project participants.  相似文献

A survey was conducted to follow up the activities of a national project to improve the quality of textbooks available in American schools. The survey found that reform efforts have affected textbook policy in the adoption states throughout the country; nonadoption states, for the most part, have not addressed problems with textbooks. Changes in the adoption states are more incremental than dramatic; more diverse than focused; more in keeping with state cultures than national in character. Certain trends, however, indicate that policy makers continue to believe that their actions can improve the textbooks used in their schools. There is evidence that the adoption processes are increasingly focused on qualitive judgments about books being made by better-prepared and better-supported selectors. In addition, the adoption processes themselves are becoming less constraining on local district decision making through a variety of mechanisms.  相似文献

Objective

This study investigated responsibilities, skill sets, degrees, and certifications required of health care navigators in order to identify areas of potential overlap with health sciences librarianship.

Method

The authors conducted a content analysis of health care navigator position announcements and developed and assigned forty-eight category terms to represent the sample''s responsibilities and skill sets.

Results

Coordination of patient care and a bachelor''s degree were the most common responsibility and degree requirements, respectively. Results also suggest that managing and providing health information resources is an area of overlap between health care navigators and health sciences librarians, and that librarians are well suited to serve on navigation teams.

Conclusion

Such overlap may provide an avenue for collaboration between navigators and health sciences librarians.  相似文献

Social media technologies are transforming the way librarians are collaborating, creating, and disseminating information. This article discusses how librarians at the University of Texas Health Science Center at San Antonio created a blog to support their health information outreach activities. Launched in 2007, the Staying Well Connected blog was established with the goal of promoting access to biomedical and health information for consumers and health professionals in the South Texas region. Between the years 1993 and 2000, the W. K. Kellogg Foundation sponsored the Comprehensive Community Health Models (CCHMs) Initiative in three Michigan counties. CCHMs was comprised of three closely related community initiatives carried out in the midst of a failed national health care reform effort and the continued penetration of managed care arrangements into many health care systems. This experimental initiative set out to test the hypothesis that traditional healthcare system animosities and exclusionary practices could be overcome by stakeholder participation in an ongoing, structured, collaborative dialogue about improving access to health services. In the process of collecting data through surveys, interviews, content analysis, and observation, we were struck by the occurrence of several overarching tensions that we perceive to exist in our data. The present article elucidates five such tensions and suggests how third parties such as communication researchers, evaluators, and practitioners can facilitate community health improvement initiatives and better their own data interpretation by acknowledging and understanding these tensions.  相似文献

The information age demands that health sciences librarians take active roles in the educational process. Librarians have traditionally taught users how to access information. Now, with the proliferation of information, librarians must accept new roles and teach the user efficient techniques for evaluating and processing information as well. Innovative roles for librarians at Texas Tech University Health Sciences Center include teaching users to use technology for information management, to appraise literature critically for quality, and to develop skills for lifelong learning. NC Health Info is an online collection of North Carolina based health services Web sites that seamlessly links local health resources to topical health information on MedlinePlus, the National Library of Medicine's consumer health information Web site. NC Health Info was the first project to connect local resources with MedlinePlus in the "Go Local" initiative. As such, NC Health Info serves as a model for other states to follow in connecting their own local information with Medline- Plus. This paper describes the processes used and lessons learned during a year-long evaluation of NC Health Info. Evaluation results may be of interest and applicable to any existing or planned "Go Local" project.  相似文献

Objectives:

This research studied hospital administrators'' and hospital-based health care providers'' (collectively, the target group) perceived value of consumer health information resources and of librarians'' roles in promoting health information literacy in their institutions.

Methods:

A web-based needs survey was developed and administered to hospital administrators and health care providers. Multiple health information literacy curricula were developed. One was pilot-tested by nine hospital libraries in the United States and Canada. Quantitative and qualitative methods were used to evaluate the curriculum and its impact on the target group.

Results:

A majority of survey respondents believed that providing consumer health information resources was critically important to fulfilling their institutions'' missions and that their hospitals could improve health information literacy by increasing awareness of its impact on patient care and by training staff to become more knowledgeable about health literacy barriers. The study showed that a librarian-taught health information literacy curriculum did raise awareness about the issue among the target group and increased both the use of National Library of Medicine consumer health resources and referrals to librarians for health information literacy support.

Conclusions:

It is hoped that many hospital administrators and health care providers will take the health information literacy curricula and recognize that librarians can educate about the topic and that providers will use related consumer health services and resources.
