目的 评估儿童慢性分泌性中耳炎手术对其父母心理状态的影响。方法 选取2017年6月～2021年10月经我科收治的因慢性分泌性中耳炎接受鼓膜置管手术治疗的患儿。患儿父母在手术前和手术后2周分别完成了20项状态特质焦虑量表1 (STAI-1)和20项状态特质焦虑量表2 (STAI-2)问卷。结果 本研究共纳入接受鼓膜置管手术治疗的慢性分泌性中耳炎患儿47例及其父母94例。术后患儿症状均明显好转。父母术前STAI-2平均得分为（29.62±7.03），其中父亲为（28.94±7.39）、母亲为（30.30±6.66），父亲与母亲的评分有统计学差异（P<0.05）。术前量表中，母亲平均得分高于父亲（P<0.05）；术后量表中，父亲与母亲的得分无统计学差异（P>0.05），但父母双方的术后STAI-1得分显著低于术前（P<0.05）。结论 慢性分泌性中耳炎患儿接受鼓膜置管手术可以导致父母有焦虑状态，尤其母亲。在对儿童进行手术治疗后，父母双方的焦虑水平均下降。必要时，可对患儿父母进行术前心理干预。 相似文献

1 病例介绍患儿男,2岁。主因有耳自幼间断流脓伴右耳后流脓二月而入院。查体:右外耳道内有脓液,拭净后见鼓膜标志不清,充血,后下象限有一小穿孔;耳后皮肤原切口处有肉芽增生,脓液流出。乳突许氏位照相示:右乳突,中、外耳孔显示致密。诊断为右慢性中耳乳突炎并耳后瘘管形成。查体无手术禁忌症,基础麻醉下行右乳突探查术。于右耳后行4cm长梭形切口,清除坏死组织及肉芽,见乳突骨质破坏,腔内有一3cm×2cm大小的胆脂瘤,确诊为右侧慢性化脓性中耳乳突炎(胆脂瘤型)。取 相似文献

目的：探讨真菌与慢性鼻窦炎伴鼻息肉发病的关系。方法：回顾分析2005．1-2005．8月在我科住院48例慢性鼻窦炎伴鼻息肉患者的临床资料。结果：48例患者真菌过敏原皮试阳性9例；鼻分泌物涂片嗜酸细胞阳性31例，真菌阳性13例，鼻窦CT窦腔实变，呈云雾状或有散在钙化点6例，其中1例上颌窦腔扩大，窦壁骨质吸收；病理检查均证实鼻息肉诊断，真菌阳性6例，变应性真菌性粘蛋白阳性4例，组织内均无真菌侵袭。所有48例患者术后随访6个月以上，有效率为93．8％。结论：真菌是鼻窦炎鼻息肉的致病因素之一，术前确诊变应性真菌性鼻窦炎非常必要。 相似文献

目的：探讨甲状腺微小癌的临床诊断和治疗方法。方法：对诊断明确的47例甲状腺微小癌进行回顾性分析。结果：术前仅12例疑为甲状腺癌．余35例均于术中冰冻和术后病理确诊。34例合并甲状腺良性疾病，伴颈淋巴结转移3例。47例中有11例术后无瘤生存时间≥10年，24例术后无瘤生存时间≥5年。结论：甲状腺微小癌术前不易确诊，且易漏诊，对甲状腺良性疾病手术时应仔细触诊，对可疑结节行冰冻切片。首次手术应行患侧腺叶及峡部切除术．有淋巴结转移者行功能性淋巴结清扫术可取得较好疗效。 相似文献

目的：探讨青光眼术后发生视网膜脱离进行激光治疗的临床疗效。方法：对8例青光眼术后发生上方裂孔的孔源性视网膜脱离的患者未采用手术治疗，而是先通过合适的体位及双眼包扎后，当视网膜下液全部或大部分吸收后采用了激光封闭视网膜裂孔的方法。结果：随访6月～1年，8例患者视网膜均复位,且未破坏青光眼手术的效果。结论：对于青光眼术后上方视网膜裂孔的孔源视网膜脱离，通过合适的体位及双眼包扎后，当视网膜下液全部或大部分吸收后可采用激光封闭视网膜裂孔，不用手术治疗，以避免因手术治疗而破坏抗青光眼手术的效果。 相似文献

目的：探讨巨大甲状腺肿的外科治疗。方法：回顾分析2005年6月至2008年11月28例巨大甲状腺肿的治疗,其中11例行一侧甲状腺腺叶切除加对侧次切术;9例行单侧甲状腺腺叶加峡部腺叶切除术;双叶甲状腺次全切除术6例;2例行全甲状腺切除术,术后长期口服甲状腺片;其中2例并行同侧颈淋巴结清除术。结果：本组28例巨大甲状腺肿的手术切除,无手术死亡病例。所有病例均见甲状腺表面血管增粗、迂曲、扩张,有的甚至形成网状静脉窦。有囊性变或囊内出血19例;有钙化灶的7例;有癌灶的2例。术后病理诊断：单纯性甲状腺肿3例,结节性甲状腺肿20例,结节性甲状腺肿伴甲亢3例,甲状腺癌2例。结论：在巨大甲状腺手术时,术前检查的准确评估,术中操作技术的精细化和不断改进,甲状旁腺自体移植技术的开展,巨大甲状腺肿的手术治疗会更安全、合理。 相似文献

髋臼边缘压缩骨折一般多伴发于后壁骨折伴髋关节后脱位,也可发生于后柱伴后壁或横断伴后壁骨折,临床还发现一种边缘压缩骨折类型,即髋臼后壁或后柱骨折并脱位后,继发的暴力造成股骨头进一步对后缘皮质骨的压缩。一般正位及斜位X线平片很难判断此种骨折存在,只有在CT片上才能看到。手术中要对压缩部分进行撬起植骨,恢复关节面的解剖结构,现报道如下。1临床资料1995年6月-2009年3月髋臼骨折手术215例,26例合并髋臼边缘压缩骨折,其中男16例,女2例,平均年龄38.6岁(25~57岁)。骨折部位:后壁骨折9例,后柱伴后壁骨折2例,横断伴后壁骨折7例。16例为典型的骨软骨面压缩(14例术前CT确诊,2例术中发现),2例为皮质骨压缩(1例术前CT发现,1例术中证实)。治疗:患者俯卧位Kocher-Langenbeck入路,横断骨折和后柱骨折者,先复位柱的骨折,用克氏针或专用复位钳固定;在处理压缩骨折时,首先探查并判断压缩骨折的部位和范围,清除无法修复的小软骨面,然后再对压缩部分进行撬起复位。撬起关节面软骨时要尽可能多带软骨下松质骨,以便使骨块获得较好的稳定。撬起后的缺损部分采用自体骨移植,小量移植骨于大粗隆处取骨,大量移植则... 相似文献

The problem on the geometrc inequalities involving an n-dimensional simplex and its inscribed simplex is studied. Aninequality is established, which reveals that the difference between the squared circumradius of the n-dimensional simplex andthe squared distance between its circumcenter and barycenter times the squared circumradius of its inscribed simplex is not lessthan the 2(n-1)th power of n times its squared inradius, and is equal to when the simplex is regular and its inscribed siplex is atangent point one. Deduction from this inequality reaches a generalization of n-dimensional Euler inequality indicating that thecircumradius of the simplex is not less than the n-fold inradius. Using virtual reality for interactive design gives a designer an intuitive vision of a design and allows the designer to achieve a viable, optimal solution in a timely manner. The article discusses the process of making the Virtual Reality System of the Humble Administrator＇s Garden. Translating building data to the Virtual Reality Modeling Language （VRML） is by far unsatisfactory. This creates a challenge for computer designers to do optimization to meet requirements. Five different approaches to optimize models have been presented in this paper. The other methods are to optimize VRML and to reduce the file size.
